I ask new patients the same question almost every visit: when was your last bone density scan, and what did the number actually mean?

Most of the time, the answer is a shrug. The test happened. Nobody explained the T-score.

A T-score of -1.0 or above is normal bone density. Between -1.0 and -2.5 is low bone mass. Below -2.5 meets criteria for osteoporosis.

But the number alone doesn’t tell the whole story. It doesn’t measure your fall risk, your bone quality, or whether you have a hidden vertebral fracture. That’s why I often pair it with a FRAX calculation, to get your real ten-year fracture risk, not just a snapshot.

If you don’t know your last T-score, that’s worth asking about at your next visit, not waiting for someone else to bring it up.

Over a million ER visits a year happen because of constipation. Most people think it’s not a big deal.

In older adults, it rarely looks like constipation. It looks like sudden confusion. A fall. Loss of appetite. By the time it’s caught, it’s often already an emergency.

I saw this constantly in the hospital. A patient’s medication list, not their diet, was usually the reason. Opioids, certain blood pressure medications, iron supplements, even some allergy medications.

If the over-the-counter aisle stopped working, that doesn’t mean you’re out of options. There’s a second category of treatment most patients are never told exists.
