A lumbar flap is a type of breast reconstruction that uses skin and fat from the lower back, specifically the “love handle” or flank area, to reconstruct the breast after mastectomy.

The tissue is carefully removed along with its blood vessels and transferred to the chest using microsurgery, where the blood supply is reconnected under a microscope. Because it uses your own tissue, a lumbar flap provides a soft, natural reconstruction.

This technique can be a great option for certain patients, especially those who:
→ Are very thin
→ Have had previous tummy surgeries
→ Don’t have enough abdominal tissue for a DIEP flap

→ Want to avoid implants

When patients choose to use their own tissue for breast reconstruction, my goal is to create a reconstruction that is uniquely tailored to their anatomy, lifestyle, and goals.

Perforator flap techniques allow me to transfer skin and fat from areas such as the abdomen, thighs, buttocks, or back while preserving the underlying muscle whenever possible. This muscle-sparing approach helps maintain strength and function at the donor site while providing natural tissue for reconstruction.

There is no single "best" flap for every patient. Whether it's a DIEP, PAP, SGAP, LAP, TDAP, or another perforator flap, each option serves a different purpose. My job is to identify the approach that best fits your body and reconstructive needs.

Autologous breast reconstruction isn’t new, but it is more specialized, and not every surgeon offers it.

Because of that, many patients aren’t fully informed about it when they’re first exploring breast reconstruction options, and may not realize there are alternatives beyond the implant-based reconstruction that is most commonly presented.

The goal isn’t to push one approach over another, it’s to ensure you have a clear understanding of what’s available. When you have that knowledge, you’re in a much stronger position to choose what aligns best with your body and your goals.

Sharing an implant-based reconstruction today! I reconstructed this patient’s right breast and slightly augmented her left breast for symmetry.

This reconstruction was performed over two stages, first with a tissue expander to structure the native breast skin. This was later converted to an implant. This is a partial subpectoral reconstruction as the patient desired a tear-drop shape to the extent possible. Although we often stay above the pectoralis muscle with the implant, she also wanted to disguise implant visibility, so we chose to go partially under the muscle. Excited for her final result. ❤️

Women are often anxious about the recovery time involved in mastectomy reconstruction. I still find a lot of misconceptions exist around recovery from free flap reconstruction, including that it can be a prolonged or very difficult course. While recovery differs for everyone, it is generally a 4-6 week timeframe after DIEP flap reconstruction and may vary depending on a patient's response to surgery.

Following a DIEP flap, women are generally observed for 2 to 3 nights in the hospital, although this can vary based on individual cases. Once discharged, recovery at home is similar to most other surgeries. Light activities are encouraged daily, like walking and everyday activities at home. The first follow-up occurs roughly one week later, drains are removed, and incisions inspected for healing. Between weeks one and four, there is a progressive increase in activities. By week four to six, I expect full healing of incisions, and patients generally return to regular activities by week six, full exercise by week 6-10 (depending on the activity), and return to work between weeks four and eight, depending on occupation. As an outpatient surgery, final revisions are conducted three and six months later to reach the final result and best aesthetic outcome.

Healing from a DIEP flap will vary depending on numerous factors. It is an exciting time as well, when I really see women gain control in their reconstructive journey and restore a sense of self after breast cancer.
