Revision series #4

Botched lipo. This is what I see in my office every day - liposuction performed rushed, uneven with disastrous consequences. This is a young woman in her 20’s who wanted some body contouring and ended up with asymmetry, deep dents and severe irregularities. To repair her I had to do multiple surgeries - a tummy tuck, two rounds of revision liposuction with fat grafting since some of the dents were so deep, and then filler to the superficial etched in wrinkles.

After all those revision procedures she was able to feel whole again but she should never have had to go through all of this. Beware of liposuction factories where cases are stack back to back or their marketing sounds too good to be true. Ask your surgeon how many patients he or she operates on a day, and look at long term before and after photos, not just ones on the OR table that are swollen and don’t show the real irregularities. Look for cannula marks or hyperpigmented scratches and also signs of photo shopping. Look for the areas the skin is stuck down to the muscle or has abnormal texture which is a sign of injury especially the lower back.

Liposuction should be performed in a way that looks natural, smooth, and where all areas blend seamlessly into each other. Please be careful, you only have one body.

Revision tummy tuck, lipo of the mons p***s with p***c lift. This patient had a tummy tuck done by a prominent surgeon in Beverly Hills - he also did breast aug w/lift and lipo at the same time - all rushed and needing to be revised. There are large dog ears, uneven lipo with shelves of fat around the bra line and hips, fat and drooping of the mons, belly button pulled into a long slit.

She had significant scar tissue with uneven internal sutures, and it was challenging to make her belly button look more natural since he had made the initial hole so large and long. She still needs a second surgery to even out the lipo as well as a full revision of her breast implants and lift.

Too often I see this - too many surgeries stacked on a patient the same day, all done rushed, sloppy. I have had to operate on SO many patients 3 times to fix one day of bad work. It may seem convenient to get everything done at once and a lot of doctors like to upsell procedures but my goal in surgery is to operate carefully, meticulously and make everything as perfect as possible with the easiest recovery and no complications. You only get one body in this life so don’t compromise and please don’t make decisions you will regret.

This was one of the worst breast implant revisions I’ve had to repair. One implant was over the muscle one was under and she developed severe scar tissue - capsular contracture- her breasts were hard and painful, the muscle had been split in half without proper elevation and one was nearly torn off the chest. To repair this patient I performed a complete capsulectomy to remove all the hardened scar tissue and implants , repaired the muscle, reshaped the pocket with sutures and lined it with Strattice and created the natural full symmetric look she desired.

This patient had gotten her implants done by a popular plastic surgeon who boasts that his surgeries take only 15-20 minutes and has done thousands of breast implant surgeries - stacking them back to back 5-6 a day. I’ve never understood why surgeons take pride in rushing surgeries - it takes me 15-20 min just to close the 4 cm breast incision in layers making a perfect scar - I can’t imagine taking 15 min start to finish to do a precise, perfectly symmetrical, minimal to no bleeding, gentle and meticulous breast implant surgery. I also perform additional steps like cauterizing every blood vessel before cutting it so there is no bleeding, making a perfect pocket and testing the shape with temporary sizers many time before putting in the final implants and sitting my patients up and down and looking from different angles, using a special sterile funnel to place implants with a no touch technique - these all take time. Ask your surgeons how many surgeries they squeeze into a day and how much time they take to do your surgery. Make sure they will do your surgery with precision and care and not see you as just a number. You deserve more.

This month, almost every surgery I am doing is a revision - liposuction revision, breast revision, tummy tuck revision, labiaplasty revision - all surgeries that if done correctly with skill and care the first time, would not have subject the patient to a second and sometimes third round of operations and the emotional devastation of having suffered a disfiguring botched surgery.

These botched surgeries were done by well known and often “Instagram famous” surgeons and patients unknowingly got swept up by the perfectly curated before and afters as well as by paid celebrities that endorsed them. Yes, complications can sometimes happen outside a surgeon’s control, but for many cases it is obvious that the surgeries were done rushed and without care. When I operate, I pay attention to every single detail, every millimeter, every suture, and I by all means I don’t expect every surgeon to be as extreme - but the degree of lack of care with which these surgeries were done is hard to comprehend. I feel obligated to share the dark side of what can happen with plastic surgery gone wrong to protect those of you who are considering surgery.

This will be the first in a series of revisions. This patient had liposuction resulting in severe irregularities, scar tissue and injury to her muscle fascia, as well as deep dents from the skin to the muscle. I performed a revision tummy tuck to help re-drape the scarred tissues, repair and tighten her muscle fascia, excise the injured skin. She is 3 mo post op. She will need a second fat grafting surgery to make the tissues even smoother. She may need camouflage tattooing for the vertical incision that was her old scarred belly button if it doesn’t fade over the next year. Prague feels like a city where every corner is holding onto a story. It feels almost like stepping into a fairytale with gothic spires, old stone bridges, cobblestone streets and views that look like they belong in a storybook. But my favorite part might be Prague’s darker charm that hide centuries of myths, legends and ghost stories. I only wish we had more time here to explore it all. 🇨🇿
