🔎 Cuffing in Lipedema

Have you ever noticed the cuff that forms around the ankles or wrists in lipedema? This a sign of underlying fibrosis.

Fibrosis creates a dense, circumferential band around the ankles and wrists that acts like a tether. This band restricts normal tissue movement and can impair lymphatic fluid flow.

🩺 I am truly honored to have testified before the US Senate Special Committee on Aging as part of “The AI Deception Machine: Deepfakes, Chatbots, and the New Frontier of Senior Fraud” bringing a healthcare lens to a growing threat facing seniors and patients everywhere.

Last year, a deepfake scam used both Dr. Herbst’s and my image and voice to falsely promote a fake lipedema cream. Our patients were misled by AI-generated content that appeared authentic and exploited the trust they place in their healthcare providers. We immediately alerted our community and worked to have the fraudulent ads, social media accounts, and website removed. Today’s testimony was an opportunity to bring attention to this growing threat and advocate for stronger protections to help prevent patients, seniors, and other vulnerable communities from becoming victims of AI-driven scams.

AI-generated scams don’t stop at one industry. As this technology evolves, so must our safeguards to protect healthcare, finance, and every corner of daily life where seniors and vulnerable populations are targeted.

We need to ensure AI technology serves people safely, ethically, and responsibly, today and for generations to come.

🩺 Not all lipedema surgeries produce the same outcome.

This patient came to us after a previous procedure with another surgeon that did not fully address her lipedema. The treatment was incomplete, leaving behind untreated areas, significant contour irregularities, scar tissue, and fibrosis.

Our revision plan is comprehensive. We’ll treat the entire affected area as one unit, carefully release scar tissue, use advanced liposuction techniques to address the remaining lipedema tissue, VASER technology to soften fibrosis, and J-Plasma to improve skin tightening and contour. By restoring a smoother foundation and using the remaining healthy fat to improve skin irregularities, our goal is to create a more natural, balanced result.

Stage 2 Lipedema Transformation ✨

This patient presented with Stage 2 lipedema affecting the thighs, pelvic girdle, torso, and upper arms, with secondary involvement of the calves and ankles.

She underwent a carefully planned, multi-stage surgical approach targeting the torso, thighs, and upper arms, with J-Plasma skin tightening added to the thighs and arms.

Now she has improved body contour, greater definition throughout the torso with visible abdominal musculature, slimmer thighs with enhanced muscle tone, and more sculpted upper arms.

Although the calves and ankles were not directly treated, they showed less swelling and improved definition after surgery. By reducing congestion in the areas above, lymphatic drainage can improve throughout the limb, leading to secondary improvements in untreated areas.

🗣️ When does lipedema start?

Most people associate lipedema with puberty and for good reason. The condition commonly begins between ages 9 and 13, often appearing around times of significant hormonal change.

However, some individuals show signs of lipedema as early as ages 5 or 6, while others may not notice symptoms until their 40s, 50s, or even later in life.

Lipedema is believed to have a strong genetic component, but its expression can vary widely from person to person. Some patients have subtle symptoms for decades, while hormonal events such as pregnancy, menopause, or gynecological surgeries can trigger a sudden progression or make previously mild symptoms much more noticeable.

This variability is one reason lipedema is so often misunderstood and underdiagnosed. Understanding when and how symptoms develop is an important step toward earlier recognition and treatment.

Transformation 🦵

After traveling from Illinois for treatment, my Stage 3 patient underwent a surgery to her calves and ankles.

As a healthcare professional, the pain, swelling, and heaviness associated with lipedema made long days on her feet increasingly difficult and affected her mobility. Having watched both her aunt and older sister struggle with lipedema, she recognized the signs and sought treatment before the condition progressed further.

🥼 Skin tightening is not one-size-fits-all in Lipedema surgery. It depends on the degree of skin elasticity and each patient’s unique needs.

For patients with good skin elasticity, especially in areas like the calves and ankles, additional skin tightening is often unnecessary. In cases of moderate skin laxity, radiofrequency technologies such as Renuvion J-Plasma or Quantum can help enhance skin retraction and support smoother results.
