This woman came to Dr. Steinsapir seeking reconstruction of her lower eyelids after developing lower eyelid retraction following transcutaneous lower blepharoplasty and lower eyelid fat grafting. She had already undergone two unsuccessful attempts to repair the problem.
Patients like this present one of the greatest reconstructive challenges because previous surgeries often consume or damage many of the tissues needed for successful reconstruction. As a result, achieving a definitive repair becomes significantly more difficult. The goal is to restore both function and appearance while avoiding additional aesthetic complications.
For example, skin grafting is sometimes offered in these situations. Although it can effectively lengthen the lower eyelid, the graft rarely provides an ideal color or texture match. While it may improve eyelid position, the aesthetic tradeoff is unacceptable for many patients.
Her treatment involved staged reconstruction by Dr. Steinsapir, including a vertical internal cheek lift to recruit skin and soft tissue into the lower eyelid, placement of a custom-made ePTFE orbital rim implant, and a hard palate graft. The procedures were performed one week apart because each eyelid needed to be sewn closed for approximately one week to optimize graft healing.
She is now one year after surgery and has experienced a profound improvement in lower eyelid position and function. Although the reconstruction was highly successful, it is important to understand that reconstruction is not the same as restoration. Her eyelids could not be returned to the state they were in before any surgery. Prior unsuccessful procedures often deplete the very tissues and structural support needed to achieve the most natural result.
Simply shortening a compromised lower eyelid—a common surgical approach—does not address the underlying problem. These eyelids are typically deficient in both the vertical and horizontal dimensions and require comprehensive reconstruction.
It can be difficult for patients to identify surgeons with extensive experience in this highly specialized area. A major focus of Dr. Steinsapir's practice is the reconstruction of complex eyelid deformities following cosmetic blepharoplasty. He has developed and refined reconstructive techniques that have helped patients from around the world who are seeking correction of these challenging problems.
Every case is unique, and the optimal reconstructive plan depends on the anatomy, prior surgeries, and remaining tissue available. There is no substitute for an in-depth, personal consultation.

From a patient who had filler service with Dr. Steinsapir:

I really enjoyed my visit yesterday. It was great to see you and Dr Steinsapir again. I am really overjoyed with the outcome. I look absolutely amazing today.

My face looks absolutely transformed!

Dr Steinsapir addressed so many of my concerns — my nasal sidewalls, both tear troughs with his beautiful technique, rebuilding my right cheek and temple, a delicate touch above my upper lip bow, smoothing the areas of my cheeks where the prior surgery had pulled too tight, and of course his signature patented Microdroplet Botox Lift along the brows. The results are simply incredible.

What moves me most is not just his extraordinary skill as an injector — though that speaks for itself — but the way he approaches every appointment. He took nearly three hours with me, listening carefully to each of my concerns and addressing them one by one. He never rushed me, and he always gave me the choice of whether to proceed with additional product or wait — that kind of respect for the patient means everything.

He is a true artist. The care he takes to make my face look as symmetrical and youthful as possible, working around the unique challenges my face presents, is something I simply have not experienced with any other doctor. He is authentic, skilled, and genuinely caring — a rare combination.

And Faith — I want you to know how much I appreciate you personally as well. The warmth and care you bring to every interaction makes such a difference. Having someone like you looking after me makes the whole experience feel truly special from start to finish.

Please let Dr. Steinsapir know how deeply I appreciate him. I feel like myself again.

With much gratitude,

This 45 year-old had lower eyelid surgery in the UK approximately 5 years ago. Unfortunately, surgery pulled his lower eyelids down. He had revisional surgery about 1 years later and was persuaded to have work on the upper and lower eyelids. Not only were the lower eyelids unimproved, but now he also felt the upper eyelids were heavy and skeletonized. He came to have an initial consultation with me about 1 year after the second surgery. Both the upper and lower eyelids were compromised but the upper eyelid findings were consistent with very aggressive surgery leaving limited resources for reconstruction. The first set of surgeries were directed at reconstructing the lower eyelids with a midface lift, a custom hand carved orbital rim implant, and a hard palate graft sewn into the back side of the eyelids. He is three years out from this work. The uppers were addressed subsequently. He has had reconstructive crease lowering surgery with anterior levator resection
surgery. Due to his prior aggressive upper eyelid surgery, his right upper eyelid benefited from the internal placement of an amniotic membrane graft.

In the before, this patient is very disappointed with her blepharoplasty result. She feels the eyes do not look like her and that the surgery took away her upper eyelid fold. Her original surgeon told her it was a great result. She consulted a couple of very well known oculoplastic surgeons who told her there was nothing that could be done. Dr. Steinsapir has pioneered upper eyelid fold reconstruction after cosmetic upper blepharoplasty. This is a two year follow-up for her post-upper blepharoplasty syndrome (PUBS) repair with Dr. Steinsapir. You do not need to live with a result you are not happy with.

Earlier this year, I saw Dr. Steinsapir and Faith for a very difficult issue involving facial filler that had left me feeling betrayed by my previous injector and unsure who I could go to for help. From the moment I began contact with their office, I felt safe, heard, and cared for in a way I’ve rarely experienced in healthcare. Their kindness, expertise, and genuine compassion stood out immediately. They took the time to truly listen, explain every step of the process, and tailor their approach to my needs. I never felt rushed or like “just another patient.” While my journey has been complex, the care I received here gave me hope and peace during a very challenging time. My results have continued to improve, and I’ve regained confidence that I wasn’t sure I’d ever feel again. In an industry that can so often feel overwhelming and uncertain, Dr. Steinsapir and Faith are a rare gift. They combine incredible skill with a level of empathy and integrity that is hard to find. I will always be grateful to them for the way they guided me through one of the most difficult experiences of my life.

Hyaluronic acid fillers are great but too often neglected. Unless removed with enzyme these products persist indefinitely. As they age, the lower G prime fillers will drift. As they age, all hold more water than when first placed. I find it helpful to remove old fillers every 3 to 4 years and start over. Some clinicians have incorporated ultrasound to localize old fillers. This is more for show as the ultrasound is really not very helpful. It is far more accurate to apply a discerning eye to guide filler removal which is as much art as science. My patient in the photos had two rounds of full face enzyme (hyaluronidase) before being refilled.
