Case from yesterday. 71 y/o retired physician with h/o rheumatoid arthritis operated multiple TAR (with the last one done five years ago) comes with the painful ankle for the last three weeks. ESR -55/ CRP -153
He has been on chronic suppressive antibiotics for the previous two years.
What’s next? I agree with the comments from all of my friends about below the knee amputation. It is the best surgery in several failed Foot Ankle procedures in patients with comorbidities. In this case, Hardware removal and antibiotic spacer insertion was performed. I’m not planning any more reconstruction like revision, total ankle replacement, or ankle fusion. In low activity rheumatoid patients, I have a reasonable success rate of treating them just with the antibiotic spacer after removal of infected total ankle replacement as long as the outcomes and the consequences have been discussed with the patient possibly it could be a limb saving surgery. Time will tell more, but I will keep you guys updated with it.

Another success story of our stellar research fellow Sung R Lee

Sung Lee, UAB SOM alumni, was one of the stellar research members of Dr. Shah’s research team at UAB Orthopedics. He worked within his team from the beginning of the first year of medical school. He matched his first choice of the orthopedic residency program. Currently, he is a 3rd-year orthopedic resident at Albert Einstein/Montefiore Medical Center, New York. His hard work, persistence, outstanding research capabilities earned more than dozen publications, podium presentations, and posters at several national and regional meetings during his tenure at UAB.

Sri is a second year medical student who has been doing research with us this past year. He is always dependable to bring solutions, ideas, and efficiency to our projects. He is a hard worker and we look forward to the bright career he has ahead of him!

Dr. Evan Sheppard, MD, was the first-ever full-time research fellow of our Orthopedic research fellowship program at UAB. Since then, our research program has grown from one research fellow to eight full-time research fellows per year for the department of Orthopedics at UAB.

Dr. Sheppard practices at Pediatric Specialist of Virginia as a pediatric orthopedic surgeon. During his research fellowship year, Dr. Sheppard was able to impress the rest of the department of Orthopedics at UAB and was hired as an orthopedic resident. We still remember for his witty jokes, magic tricks, outstanding research capabilities.

Introducing: Felisa
Felisa just celebrated her 2 year anniversary with the department as an Office Service Specialist. She is always a team player, a problem solver, and an innovator! When she isn’t working she is the road trip queen- she loves to travel and the beach is her favorite place. She also loves being a “Gigi” to her 2 grand daughters, with another on the way next month!

We are always looking forward to our weekly foot & ankle research meetings! We discuss our current research and brainstorm more ideas with this vibrant group of research fellows, which sometimes include ideas for instagram posts 😄😄. We are lucky to have this intelligent group of people working with us.
