8 hours in bed is not the same as 8 hours of sleep.

The sleep position series — Part 2.

Your brain cycles through five stages of sleep. Stage 3 — deep sleep — is when your body repairs and your brain clears metabolic waste. You can spend 8 hours in bed and get almost none of Stage 3.

As a whole-body dentist, I used to tell exhausted patients to sleep more. More hours is not the answer if the hours are not deep enough. When your airway partially obstructs at night your brain rouses you from deep sleep to restart breathing. You do not remember it. It happens dozens of times per night. You get the hours. You do not get the depth.

That is why you wake up exhausted after a full night.

Estrogen plays a direct role in maintaining muscle tone in the airway. As estrogen drops in
perimenopause, airway muscle tone drops with it. The airway narrows. Sleep fragments. The symptomslook hormonal. And they are — partially.

Night guards protect your teeth. They do not stop the clenching. They do not fix why you clench. And for some patients the wrong night guard makes the jaw position worse over time.

As a whole-body dentist, I had a patient who had worn through four custom night guards in two years.

Forward head posture does not stay in your neck. For every inch your head moves forward from neutral, it adds 10 pounds of load to your neck and jaw muscles. Those muscles attach to your jaw joint. When they are overloaded the joint compensates and suffers.

As a whole-body dentist and a dad in 2026, I watch my daughters on their phones and I see the chain reaction starting. A 28-year-old came in last month with TMJ, chronic neck pain, and daily headaches.

If you think an airway dentist works alone, you are thinking about this wrong.

The whole-body team series Part 1.

Here are the four people I believe are essential if you are serious about airway health. And I will be honest, I am not the most important one on this list.

Number 1: An airway dentist. I coordinate the dental side. I am the quarterback on that half of the field. Not the whole game.

Number 2: A board-certified sleep physician. Mine is an internal medicine doctor. He prescribes and reads every home sleep test. He diagnoses sleep apnea. I cannot do that. He can.

Number 3: A board-certified ENT. Your airway is not just your mouth. It is your nose, your throat, your entire upper respiratory system. An ENT sees what I cannot.

Number 4: A myofunctional therapist. Once the structure is addressed, you have to retrain the muscles. Without this step, the results do not hold.

And apparently Number 5 is a patient like mine who goes viral posting about her treatment. That one I did not plan. But I will take it.

ADHD and sleep apnea in children share almost every symptom: difficulty concentrating, hyperactivity, emotional dysregulation, and poor memory. Most children are treated for one when the other is the actual cause.
