Nobody warns you what losing weight fast does to your face.

This is Jerry, 62. She had surgery a couple weeks ago, and the weight came off fast. Here’s what most people don’t know: your face is held up by fat. Deep pads in the cheeks and midface work like scaffolding for everything around them. Lose weight quickly and those pads deflate faster than your skin can retract so the support collapses. The cheeks flatten, the midface hollows, the lower face gets heavier, and you’re left with under-eye shadows, jowling, and that dip in front of the chin (the pre-jowl sulcus). The face doesn’t just get thinner. It loses its shape going from a lifted V to a heavier, squared-off look. And past 40, it happens faster, because the aging is already underway.

Threads don’t replace volume. They reposition tissue. That’s the part most people get wrong.

A thread moves what’s already on your face it can’t put back what your face lost. So when threads go into a deflated midface, there’s nothing underneath to hold the lift. The tissue has nowhere to settle, the lift gives out, and you’re told threads “just don’t last.”

They last fine. They were asked to do a job they were never built for.

Threads and filler aren’t competing options they do different jobs. Filler restores the volume the face lost and rebuilds the foundation. Threads redirect the tissue back to where it belongs. One without the other is why so many lifts give out.

The lift holds because it was never asked to do the job of volume.

A lift only works on a supported face. Threads are one step inside the plan, never the plan itself.

Looking untouched isn’t about restraint. It’s about who’s holding the needle.

An infraorbital hollow is usually a midface problem. When the deep medial cheek fat and the SOOF lose volume, the lid-cheek junction lengthens and a shadow forms below it. An injector who treats the shadow is treating a symptom. An injector who understands the anatomy treats the cause.
So we sequence it. Deep midface first supraperiosteal, structural. Once the cheek holds again, the shadow lifts on its own and the under-eye needs very little. Then the lower face: chin at the pogonion for anterior projection, shadowing to define the mandibular border. Width becomes structure instead of fullness.

Anyone can inject. Knowing what to correct, in what order, and at what depth is the difference between looking done and looking like yourself.

Paint one wall of a room and stop. The new wall isn’t the problem. It just makes every wall next to it look dingy. A face reads the same way correct one area and you haven’t finished the job, you’ve drawn attention to the rest.

Here’s why that happens anatomically. The cheek is built from separate fat compartments held over bone by retaining ligaments. With age the maxilla and orbital rim resorb, those ligaments lengthen, and the compartments migrate down and in. The fat didn’t vanish. It moved. Hollow above, heavy below. Same fat, wrong address.

So the fold you keep filling is a shadow cast by tissue that left its shelf. Fill the shadow alone and the lower face gets heavier, not lifted.

This patient presented with one structure losing support in four places at once: infraorbital hollowing, midface fat pad descent, nasolabial folds, pre-jowl sulcus. We treated six areas in the order that rebuilds support under-eye, cheek, smile line, chin, lip, PDO threads. Shortened the philtrum instead of inflating the lip. Projected the chin so the mouth had something to sit against. Rebuilt the cheek so its own curve reappeared.

Nobody should be able to tell you had anything done.

When they can, it’s almost never because too much product was used. It’s because it was placed in the wrong plane.

Here’s what actually creates that filled, puffy look: injecting superficially into the tear trough without deep structural support underneath it. There’s a name for the result in the literature. It gets worse when you smile, because there’s nothing anchoring it.

Alonso, 35, had wanted the under-eyes corrected for years, but never at the cost of looking done. With a naturally fuller lower face, filler felt like it would only tip into puffy. That fear was well-placed it just pointed at the wrong structure.
