Brain injury recovery does not happen in isolation, and support should not end at diagnosis or discharge. The Continuum of Care for Neuro-Resource Facilitation Program is designed to help brain injury survivors, their families, and caregivers navigate recovery with personalized support every step of the way.

Through goal planning, resource identification, referrals, service monitoring, and ongoing adjustments, this program helps New Yorkers access the supports they need to move toward meaningful outcomes and greater independence.

The program also empowers participants to advocate for themselves while connecting them with timely, comprehensive, and tailored resources in their communities.
