Shoulder arthritis can be debilitating. Shoulder replacment has excellent results to relieve pain. Different types of shoulder replacements are available to address many of the sources of shoulder arthritis. If you have an intact rotator cuff, best option is an anatomic replacment to regain near normal pre-arthritic motion and function.

Not every shoulder replacement should be a reverse. The reverse replacement is the workhorse in the shoulder but is not always the best option.

Know your options.

ACL tears are a very common knee injury from young athletes to weekend warriors. The gold standard has been ACL reconstruction where we use a tendon (quad, hamstring or patellar tendon) to recreate the ligament. Even with the reconstruction, we have accepted the fact of early osteoarthritis changes still occurred even though the knee was stabilized. Now the pendulum has shifted—ACL repair with a scaffold. The biologic repair has similar results of success as the reconstruction but now the repair is setting the standard by preventing the previously accepted post injury osteoarthritis. Time to consider the future. Time for your ACL repair.

after shoulder replacement can happen for several reasons. This is an example of an “overstuffed” replacement. The first surgery failed to recreate the center of rotation due to an oversized head thickness/lack of proper resection. Glenoid loosened, subscapularis tendon failed. Shoulder remained painful and lost function. This is not an implant I use but in this scenario I maintained the humeral stem with its convertible design to avoid further injury attempting to remove a large pressfit well seated stem. Fortunate stem was located where I could convert to a well positioned reverse. Video is from 2 years after surgery converting the failed anatomic to a reverse.
