In part four of our palliative criteria series, we cover malnutrition.

At Angels Grace Hospice & Palliative Care, the focus is to recognize and help manage symptoms with the collaboration of primary team, improve comfort, and help patients maintain the highest possible quality of life. Every care plan is individualized, recognizing that nutritional needs and treatment goals differ from one person to another.

The Angels Grace Hospice & Palliative Care APNs work together in collaboration with primary care teams to address underlying causes. By managing pain, nausea, dry mouth, swallowing difficulties, constipation, and other symptoms that interfere with eating, the team helps patients enjoy meals more comfortably.

A stroke, also known as a Cerebrovascular Accident (CVA), can change life in an instant — affecting speech, mobility, memory, swallowing, independence, and emotional well-being. Whether someone is recovering from a recent stroke or navigating long-term complications, at Angels Grace Hospice & Palliative Care, we offer an added layer of support focused on comfort, symptom management, and quality of life. Our goal is to help patients and families better understand what to expect while creating a plan of care centered around each person’s unique needs, goals, and wishes.

In part two of our palliative criteria series, we cover ALS (Lou Gehrig’s Disease) patients.

Angels Grace Hospice & Palliative Care of Illinois provides specialized palliative care for patients with terminal illnesses, including Amyotrophic Lateral Sclerosis (ALS), also known as Lou Gehrig’s Disease.*

ALS (Amyotrophic Lateral Sclerosis) is a progressive neurological condition that affects nerve cells responsible for muscle movement, speech, swallowing, and breathing. Palliative Care for ALS focuses on improving quality of life by working collaboratively with the patient’s medical team to help manage symptoms and reduce discomfort. Palliative Care is instrumental in providing emotional and practical support for both patients and families.

Palliative Care for ALS works in collaboration with neurologists and other specialists to help patients navigate complex decisions, coordinate care, and manage changing symptoms over time. Support may include working with the patient and medical team to help with pain and symptom management.
