There’s encouraging news for those of us navigating Medicare: the Centers for Medicare & Medicaid Services (CMS) is taking steps to make healthcare more affordable. A new proposal aims to reduce the payment differences between hospital outpatient facilities and independent offices for imaging services. By expanding what’s known as site-neutral payments to all outpatient services, CMS expects to lower overall costs, encourage healthy competition, and save billions.

Every September, Medicare Advantage and Part D members receive what may be the single most important piece of mail of the year: the Annual Notice of Change. Too often, folks just toss it aside without a second glance. But inside, you’ll find details about increases to your Part B premiums, updates to deductibles, and other plan changes that could affect your coverage and costs. I’ve always believed that Medicare should adapt to your life—not the other way around. Recent trends are showing just how much beneficiaries are taking that to heart. From 2016 to 2022, there was a notable rise in voluntary Medicare Advantage plan switching—especially among dual-eligible enrollees. For fully dual-eligible individuals, switching rates within Medicare Advantage jumped from about 12% to nearly 22%. Partial dual-eligible saw an even bigger shift, moving from around 17% up to 30%. Other beneficiaries experienced a more modest increase, from 10% to 13%.

Interestingly, making the leap from Medicare Advantage back to traditional Medicare remains rare: just 4% for fully dual-eligible, 2% for partial dual-eligible, and less than 1% for others. The data also highlights differences by age, risk level, and race or ethnicity. Older beneficiaries tended to stay put, while those with higher health risks were more likely to consider traditional Medicare. Meanwhile, those enrolled in dual special needs plans were less likely to switch at all.

Many people are surprised to learn that, while life insurance death benefits are usually tax-free, any interest earned on delayed payouts can be taxable—and that extra income can affect your Medicare costs. For example, if a widow typically has a MAGI around $105,000, an additional $10,000 in taxable interest could move her above the $109,000 threshold for standard Medicare Part B premiums. Once MAGI reaches about $115,000, single filers see their Part B premium increase by $81.20 per month and Part D by $14.50, adding up to roughly $1,100 in extra yearly premiums.

After the loss of a spouse, it’s worth knowing that you may be able to appeal for lower premiums if your household income has dropped. Medicare will sometimes review a newer, lower income instead of relying on an older tax return.
