The BVM isn't going anywhere.
At FDIC International 2026, Mike Brown of JEMS opened the panel's discussion of mechanical ventilators with a reality check: "We're always going to need to have good, effective use of bag-valve-mask ventilation."
A mechanical ventilator won't be in the hands of every prehospital provider who meets a patient in sudden cardiac arrest. A BVM already is.
So the real question is how to make every bag breath a good one.

What did our last workshop tell us about manual ventilation training?
138 attendees answered our live polls at the September 22 "Mastering Flow Control in Manual Ventilation" workshop.
📊 75% said their service runs manual ventilation booster training less than twice a year. That includes 20% who said never.
📊 71% said their service does not use gamified training.
The 2025 AHA Guidelines recommend booster sessions after single-block resuscitation training, and say gamified learning may be reasonable.
The good news? 80% correctly answered our dead-space question (about 150 mL in an average adult). That's our second-highest score to date.

Why does 1 second matter when you squeeze a BVM?
Same 500 mL. Different breath.

Squeeze it over 1 second and flow peaks near 50 L/min.
Rush it in half a second and flow peaks near 100 L/min.
Average flow is volume divided by time, so 30 vs 60 L/min. But a hand squeeze speeds up and then slows down, so the peak runs higher than the average.

Higher peak flow means higher peak airway pressure. That can push air into the stomach, injure the lungs and potentially cause a pneumothorax or pneumomediastinum.

The 2025 AHA Guidelines say it may be reasonable to give each breath over 1 second for adults in cardiac arrest without an advanced airway. They list both too little and too much ventilation as Class 3: Harm.
