MGH IHP COTAD Chapter members Alexandra, Valeria, Madeleine, Luke, Hanna, Betsi, and Gwen were proud to volunteer at Chelsea’s Disability Pride Month celebration, standing alongside community members, advocates, and organizations to honor disability identity, inclusion, and belonging. A special thank you to Flor Amaya, Director of Public Health for the City of Chelsea, MA and a MGH Institute Doctor of Physical Therapy Program alumna, for bringing the community together around the message that our differences make us stronger and inviting. What happens after stroke rehabilitation ends?

In this episode, occupational therapist and stroke researcher Dr. Kimberly Erler joins nurse scientist Dr. Kathryn Sabo to discuss the “post-acute care cliff”—the point where formal rehabilitation services end, but the work of recovery continues.

Together they explore:
🧠 Why stroke should be viewed as a chronic condition
🏠 The realities facing survivors and care partners after discharge
🤖 Why promising rehabilitation technologies often fail to reach everyday practice
💡 How implementation science can help bridge the gap between innovation and impact

The conversation also highlights the work of the Tedy’s Team Center of Excellence for Stroke Recovery the MGH Institute of Health Professions and the importance of interprofessional approaches to long-term recovery.
