What does "bottoming out" mean—and how can it be addressed?
Breast implants can shift downward or outward over time, particularly when changes in breast tissue, skin quality, weight, or previous surgery affect the support around the implant.
In this case, Dr. Burgdorf is performing a breast revision after a previous augmentation and lift following significant weight loss. The implants had shifted downward and outward, so the goal is to reposition them and provide additional support to the breast pocket.
One tool Dr. Burgdorf may use in appropriate revision cases is GalaFLEX®, a surgical mesh designed to provide additional internal support. He describes it as an "internal bra" because it can help support the breast and implant from within.
For this patient, the goal is to create more upward and inward support, improve implant position, and restore the projection and proportions she wants.
