Bay Mills Community Health The Community Health team focuses on preventing chronic illness and helping those living with chronic illnesses.

The Community Health team at the Bay Mills Health Center focuses on preventing chronic illness, patient education, maternal-infant home visiting programs, and seasonal community immunization clinics. It is also our goals to care for the overall health and understanding of healthy living by community members. Our services include, but are not limited to:
Home health care (Arrangements made with CH)

SNAP Update: No new SNAP benefits are being issued in November due to a pause in federal funding.

✔️ Spend previously earned Double Up bucks — no expiration
