Lets not forget !!

If your child will be participating in physical activity, i.e. band, color guard, sports, etc., please make sure you bring in the school physical form supplied by the school before your visit . Be sure to prefill the form by answering the questions regarding your child's health . Keep in mind that your child's health insurance plan may or may not cover the cost for a physical.
