Some treatment-resistant cases aren't treatment-resistant at all.

According to Jay Gunkelman, many medication failures may be tied to localized brain dysfunction that requires a different approach entirely.

"If you have focal slowing, this isn't normal psychiatric stuff."

Could an EEG reveal what's being missed?

What do you think?

YOU CAN'T GIVE SOUP TO THE WHOLE BRAIN

What happens when the real problem sits in one small spot — but the treatment hits everything?

This week Jay Gunkelman walks a before-and-after EEG case where a localized brain issue was hiding behind symptoms that usually get treated systemically. You can watch the brain settle from one session to the next.

In the episode:
🧠 Why some "medication failures" aren't medication failures at all
🧠 What an EEG reveals when symptoms won't improve
🧠 Local brain dysfunction vs. a whole-brain fix — neurofeedback, TMS, hyperbaric
🧠 Treatment-resistant cases, EEG biomarkers, and clinical decisions
🧠 Sleep, attention, and whether schools should screen brain function earlier

If you've ever wondered why some people don't respond to conventional treatment, this one offers a different look at what might actually be going on underneath.

Should EEG be used more often when traditional treatments aren't working? 🧠

Whether you have kids in school or not, you're paying for this.

Some districts spend about $17,000 per student. Others spend roughly $32,000. Plenty spend far less.

But the dollar amount isn't the real problem. It's that we spend serious money to develop young minds and never measure the one organ doing the learning — rich district or poor.

DIGITAL BEER 🍺🧠

Can you create your own alpha waves without alcohol?

In this NeuroNoodle clip, Dr. Jay Gunkelman explains how some people can learn to generate alpha activity naturally—even if they come from families with a history of alcoholism.

"Digital beer. Digital beer."

A short clip from: "The Cingulate Doesn't Sleep"

Joshua Moore bet his car on a left posterior concussion.

Then Jay Gunkelman looked at the EEG blind — age only, no history — and immediately went somewhere deeper:
the anterior cingulate.

This 33-second clip is the exact moment the case split in two.

Raw EEG analysis. No script. No prep. Just pattern recognition from someone who has read over 500,000 brain scans.

NEW EPISODE: The Cingulate Doesn’t Sleep

Joshua Moore sent Jay Gunkelman an EEG from an 18-year-old and gave him almost nothing to work with:
age only.

No history.
No diagnosis.
No setup.

Joshua thought it pointed to a left posterior concussion.

Jay saw something else:
an anterior cingulate failure pattern with thalamocortical dysrhythmia, beta spindling above 30Hz, mu disconnect on the left hemisphere, and deeper salience-network involvement.

Then the discussion got even deeper:

insomnia
rumination
sensory sensitivity
dissociative PTSD patterns
posterior insula findings
why phenotypes may matter more than DSM labels

This is one of the strongest blind-read breakdowns we’ve done on NeuroNoodle.
