Understanding SI joint pain and why it is often misdiagnosed.

In this video, board-certified neurosurgeon Dr. Joseph Shehadi explains sacroiliitis, a condition affecting the sacroiliac—or SI—joint that can sometimes be mistaken for other causes of lower back pain.

Treatment may begin with medication and image-guided injections. For appropriately selected patients, SI joint fusion may also be considered to help stabilize the joint.

A complete medical evaluation is essential to identify the source of the pain and determine the appropriate treatment.

What does a lumbar endoscopic neurectomy involve?

In this video, board-certified neurosurgeon Dr. Joseph Shehadi explains this minimally invasive outpatient procedure, which is performed under general anesthesia using a small scope and two small incisions.

Lumbar endoscopic neurectomy may provide approximately 9 to 18 months of relief for appropriately selected patients. It can offer a middle-ground treatment option between conventional spinal injections and more extensive procedures such as lumbar laminectomy or fusion.

A complete medical evaluation is required to determine whether this procedure is appropriate for a particular patient.

When is lumbar endoscopic neurectomy considered?

In this video, board-certified neurosurgeon Dr. Joseph Shehadi explains how diagnostic and therapeutic treatments—such as facet blocks, medial branch blocks, and lumbar transforaminal epidural steroid injections—can help determine whether lumbar endoscopic neurectomy may be an appropriate next step.

Treatment decisions depend on the source of the pain, the patient’s response to prior injections, and a complete medical evaluation.

🩺 Who Is the Ideal Candidate for Lumbar Spine Surgery?

Not everyone with back pain needs surgery.

In this video, Dr. Joseph Shehadi explains who may be a good candidate for lumbar spine surgery. Generally, surgery may be considered for patients who:

✔️ Continue to experience significant back or leg pain despite conservative treatment
✔️ Have conditions such as lumbar radiculopathy (sciatica) or herniated discs
✔️ Have symptoms that are affecting their daily activities and quality of life

The decision to proceed with surgery is always based on a thorough evaluation of your symptoms, imaging studies, and overall health.

At Northtowns Neurosurgery, our priority is helping you find the treatment that's right for you—whether that's conservative care or surgery.

🩺 Avoiding Surgery Whenever Possible

At Northtowns Neurosurgery, surgery is never the first recommendation—it's the right recommendation only when it's truly needed.

In this video, Dr. Joseph Shehadi explains how many patients can improve through conservative treatment options, including:

✔️ Home exercise programs
✔️ Physical therapy
✔️ Vitamins and appropriate medications
✔️ Other non-surgical therapies, such as acupuncture, when appropriate

Every patient receives a personalized treatment plan based on their condition, symptoms, and goals. When non-surgical options can provide relief, Dr. Shehadi believes they should always be explored first.

If you've been told you may need lumbar spine surgery, you've probably heard the term pedicle screws. But what are they, and why are they used?

In this video, Dr. Joseph Shehadi explains how pedicle screws play an important role in certain spinal procedures. These specialized implants are used to stabilize the spine, helping maintain proper alignment while the bones heal and fuse together.

🦴 What Does "Spinal Instability" Mean?

If you've been told you have spinal instability, you may be wondering what it actually means—and whether spinal fusion is necessary.

In this video, Dr. Joseph Shehadi explains that spinal instability occurs when the spine moves more than it should, often leading to pain, nerve compression, or reduced function.

For some patients, a lumbar fusion may be recommended to stabilize the affected area. By using specialized instrumentation, such as screws and rods, the goal is to restore stability, relieve pressure on the nerves, and help patients return to a more active, comfortable life.

At Northtowns Neurosurgery, every treatment plan is personalized. Surgery is only recommended when it's the best option for your condition and long-term health.
