💙 We hope you never need this resource.
But we also know many of our amazing kiddos are curious, adventurous, and sometimes a little like Houdini. Taking a few minutes now to prepare could make all the difference if the unthinkable ever happens.
We encourage our Grace in Motion families to take advantage of this opportunity and share it with others who may benefit. Being proactive is one of the best ways we can help keep our children safe.💙
We are committed to helping keep all members of our community safe, especially individuals with autism and other disabilities who may need additional assistance during an emergency.
Recently, a child with autism was found walking alone. Our responders worked to identify him and reunite him with his family, but this situation reminded us just how important it is for families to complete a Residential Premise Form and submit a current photo of their loved one.
Had we already had his information and photograph on file, we may have been able to identify him and reunite him with his family much faster.
The Residential Premise Form allows us to provide first responders with important information, including:
✔ A current photograph
✔ Communication needs
✔ Likes, dislikes, and calming techniques
✔ Behavioral considerations and triggers
✔ Wandering or elopement concerns
✔ Emergency contacts and caregiver information
✔ Other information that could help us during an emergency
This program is supported by Logan's Law, and the information is reviewed every six months to help ensure it remains current.
Please do not wait until an emergency happens. If you have a child or adult loved one with autism or another disability, we encourage you to complete a form and provide us with a current picture. Having this information available can make a critical difference when every minute matters.
We also ask that you please share this information with family members, friends, caregivers, teachers, support groups, and anyone you know who has a child or adult loved one with a disability. Help us get this information into the hands of the families who may need it most.

A form and a picture may seem like a small thing, but during an emergency, they could help us identify your loved one and bring them home safely that much faster.
