The procedure mattersโbut recovery is also shaped by what happens outside the operating room. These five measurable and potentially modifiable factors can influence pain, function, rehabilitation, and satisfaction after spine surgery:
1๏ธโฃ Metabolic and nutritional health
The body heals on its reserves. Poorly controlled diabetes, smoking, obesity, low muscle mass, and nutritional deficiencies are linked to infection, hardware complications, and impaired fusion. Glycemic control, smoking cessation, and better nutrition provide concrete targets before surgery.
2๏ธโฃ Mental and emotional readiness
Untreated anxiety or depression can heighten pain, disrupt sleep, reduce motivation, and make rehabilitation harder. Tools like the GAD-7 and PHQ-9 can identify concerns that may benefit from treatment before and after surgery. Patients with depression can still improve significantly after an appropriately indicated operation.
3๏ธโฃ Commitment to rehabilitation
The work before surgery can pay off afterward. Exercise-based prehabilitation may improve strength, pain, disability, fear of movement, and early postoperative recovery. Education, physical conditioning, and an individualized plan are key.
4๏ธโฃ Support system and social environment
Recovery rarely happens alone. Limited support is associated with longer hospital stays and discharge to a facility instead of home. Confirming home assistance and planning ahead can make recovery safer and smoother.

Poor sleep can amplify pain, worsen mood, and drain the energy rehabilitation requires. Sleep can be tracked and optimized alongside the surgical planโincluding screening for sleep apnea and maintaining a consistent sleep schedule.
A difficult recovery is not the patientโs fault, and positive thinking cannot overcome structural disease.
