At the age of 14, Dr. Croley made the commitment to become an eye MD and help people with vision problems. He graduated Magna Cum Laude from Cumberland College, and attended medical school at the University of Miami. He then completed his internship at Baptist Medical Centers in Birmingham, Alabama. His eye residency was at the University of Alabama Eye Foundation Hospital, where he became Chief R

esident and House Staff Representative. In 1980, Dr. Croley moved to Southwest Florida to open his practice and has since been very active in the development of new advances in ophthalmology. He has traveled extensively across the nation and overseas consulting with other eye MDs to promote excellence in eye care. Dr. Croley has been involved in the design and development of Electronic Medical Records and Telemedicine for more than 20 years.

Not every large retinal “mass” is a tumor.

This patient presented with sudden loss of central vision. The fundus photo shows a massive submacular hemorrhage occupying much of the posterior pole.

At first glance, lesions like this can mimic a choroidal melanoma or other intraocular mass. OCT was largely obscured by the hemorrhage, emphasizing the importance of clinical history and maintaining a broad differential diagnosis.

I’ve encountered similar presentations in patients with leukemia, where hematologic abnormalities can lead to dramatic retinal and subretinal hemorrhage.

A reminder that in ophthalmology, appearances can be deceiving—and the patient’s history is often as important as the image itself.
