⏰ Don’t Wait—Refill Before You’re Out! Running out of medications—even for a day—can impact your health. Whether it’s a prescription, vitamin, or supplement, your provider needs to know what you’re taking to keep you on track.

✅ Call ahead for refills—before your supply runs out. With the new scheduling model, medication refill appointments will need to be made 3-days in advance to help guarantee "no gap" in medications
✅ Let your provider know if you’ve made any medication changes

A Community Health Worker (CHW) is a frontline public health professional who serves as a vital link between healthcare services and the communities they serve — promoting health equity and improving access to care.

During this visit, Shandi can assist with:
• Uninsured or underinsured support
• Medicaid enrollment
• Marketplace insurance
• SNAP benefits
• Additional community resources

All services are free of charge.

- Patient Access Representative
- Respiratory Therapist (PRN)
- Emergency Department and Inpatient Services Licensed Practical Nurses/Registered Nurses (Nights)
- Primary Care Clinic Licensed Practical Nurse

CCMH offers an additional $6 per hour night-shift differential and $2 per hour weekend differentials.
