Not every nurse gets the opportunity to influence vascular access decisions.

But those who do… see things differently.

They’re not just placing lines... they’re assessing the full picture: Patient condition. Therapy plan. Vessel health. Risk factors. Long-term outcomes.

And when something doesn’t add up? They speak up.

💬 “There are times when the initial plan doesn’t match what we’re seeing at the bedside. Having the confidence and support to advocate for a better option changes everything.” ~Vascular Wellness Clinician

At Vascular Wellness, we believe vascular access is more than a skillset... it’s a clinical specialty.

If you’re a nurse who wants to:
• Be heard in clinical decisions
• Strengthen your assessment skills
• Expand your impact at the bedside

Many clinicians use these terms interchangeably, but doing so can obscure an important clinical distinction.

The presence of a bloodstream infection in a patient with a central line does not automatically mean the catheter caused the infection. Yet that assumption is often made.

So how do clinicians determine whether the catheter is truly the source?

Our latest article examines the differences between CLABSIs and CRBSIs, why the distinction matters for patient care and reporting, and what healthcare organizations can do to reduce these serious infections.

A Vascular Wellness clinician arrived at a hospital ICU to place a PICC line for a patient who had already been identified as needing vascular access support. Before she could begin reviewing the case, a rapid response was called on another unit.

When the responding ICU nurse returned, he asked her to shift priorities. The patient he had just evaluated was deteriorating rapidly and would likely need vascular access more urgently than the original consult.

As the care team worked through the situation, the patient was transferred to the ICU with a heart rate approaching 200 beats per minute and a systolic blood pressure in the 60s. Then another complication emerged: the patient's only IV infiltrated just as critical medications were being prepared.

What happened next required more than technical skill. It required clinical judgment, coordination across departments, and the ability to adapt when the patient's condition changed faster than the original plan.

A Skilled Nursing Facility resident arrived in the Emergency Department with a fever of unknown origin and concern for possible sepsis. The sepsis protocol was ready to begin, but there was one major obstacle... none of the facility staff could obtain vascular access.

The patient was severely dehydrated with contracted upper extremities, and multiple traditional and ultrasound-guided IV attempts had already failed by the time our clinician was asked to assess the situation.

In suspected sepsis cases, time matters. Delays in obtaining labs, blood cultures, fluids, and antibiotics can quickly change the trajectory of care.

This case is a reminder that vascular access is never “just a task.” In critical moments, it can become the bridge between uncertainty and intervention.

What made this case especially interesting was the recommendation that followed.

Some members of the care team expected escalation directly to a central line.
