Dear Valued Patients,
It is with a full heart and a deep sense of peace that I am writing to share an important update regarding my medical practice. After much prayer, reflection and thoughtful consideration, I have made the difficult decision to close my medical practice. Our final day for seeing patients will be April 30, 2026.
A Journey I Will Forever Treasure
For the past 16 years, it has been one of the greatest honors of my career to serve as your medical provider. Over nearly two decades, many of you have become much more than patients—you have truly become a part of my extended family. We have walked through seasons of joy and seasons of hardship together. We have celebrated milestones, welcomed new life, grieved losses, and shared countless conversations that I will carry with me always.
Caring for my hometown has shaped me both professionally and personally. I am profoundly grateful for the trust you placed in me and the privilege of being a part of your lives.
The Path Forward
As many of you know, the ongoing complexities and time-consuming nature of the CMS correction process have been significant. Dealing with these administrative hurdles for as long as I have based on CMS timelines, has led me to feel a strong "pull" toward a redirection in my life. I feel called to slow down and devote more of my time to what matters most: my family, my life, and especially my grandsons.
While I am closing this professional chapter, I am looking forward to the next one and following where God is guiding me.
Medical Records & Continuity of Care
Your continued health and continuity of care remains my priority during this transition. Please take note of the following information regarding your medical records:
• Process: To transfer your medical records, please have your new healthcare provider mail a signed Record Release Form to our PO Box. Or fill out the enclosed request and mail to the below PO Box.

• Outstanding Balances: We would appreciate you calling the office or stopping by and paying all outstanding balances as soon as possible before April 30, 2026. Payments after closure can be mailed to the above address as well. This will help facilitate a seamless transfer of your medical records to your new provider. All remaining balances will be referred to an external collection agency with additional late fees added.
• Medications: We will be refilling routine medications for an additional 30 days past the April 30, 2026 date. Please have your pharmacy send a refill notice if you are in need of any medication refills.
