The STARt app is such a helpful tool for ‘r’ elicitation. I like that kids can see how the waves change when they experiment with moving their tongue around. If/when they stumble upon a better ‘r’ sound, they can freeze their tongue and focus on what it feels like. This helps them improve their proprioceptive awareness and hopefully begin to establish a new motor plan.

What do you do when a target word/phrase breaks down in apraxia therapy?

If in doubt, slow it down. Chaining is another handy trick. But if the tricks aren’t helping, simplify the target word/phrase and back up to simultaneous production/ direct imitation (if you’re using the DTTC method). What we don’t want to do is keep practicing a motor plan that isn’t accurate ❌ It’s important to pay attention to the breakdowns, and if your tricks aren’t working, simplify the task and provide more support.

Working with kids with autism and apraxia can be challenging. I’m not an expert by any means, but I’ve seen more generalization and spontaneous language as I’ve spent more time offering support/cues during play to help these kids learn when (and not just how) to say their target words.

Minimal pairs therapy with a hyper 4 year old 😂 they understand how to drill, but you can see how we still need to work on attending to tasks. I love how facial expressions and being a little silly can make minimal pairs therapy more meaningful and fun at the same time.

DIR Floortime is a play-based approach that focuses on connection and shared joy with a child. I have been so impressed with how the kids on my caseload have responded to this method. When kids are regulated and feel connected, their capacity to learn and communicate is incredible. If you work with autistic kids, I cannot recommend Floortime training enough.

Stretching out vowels prevents speech from being choppy and segmented, which is super important when it comes to natural sounding speech.

There can be a lot to focus on in apraxia therapy. Sometimes we work on vowels and then I realize we’ve lost a final consonant. Other times I realize I need to provide more support or that the type of prompts I’m using aren’t helping. I watch clips of my sessions and realize I should’ve faded my prompts sooner. But I appreciate that the kids are so patient and still show up ready to work every session. I still think this is the most rewarding field to be in ❤️

Facilitative contexts can help a lot when you’re trying to get a good ‘r’ sound. Try experimenting with different contexts to see which is best for the child’s ‘r’ and then start practicing there.
