New research looking at respiratory muscle strength training (RMST) during acute hospitalization for COPD found a promising reduction in 30-day hospital readmissions.

In this retrospective study, an SLP-led RMST program was well tolerated, with no adverse events reported. Readmissions decreased from 14.4% to 7.3% during the implementation year.

The findings are preliminary, but they raise an interesting question about whether RMST could play a role earlier in the COPD care pathway, including during hospitalization.

Definitely worth a read for anyone working with patients with COPD.

Can stronger expiratory muscles change how hard breathing feels?

A paper from 1995 that is still worth revisiting.

Suzuki, Sato, and Okubo studied 12 healthy adults to see whether expiratory muscle training could affect the sensation of respiratory effort during exercise.

Six participants completed four weeks of expiratory muscle training.

After training:

• Expiratory muscle strength increased by 25%
• Participants reported less respiratory effort at the same levels of exercise
• Minute ventilation decreased
• Breathing frequency decreased, while expiratory time increased

The authors concluded that expiratory muscle training increased expiratory muscle strength and reduced the sensation of respiratory effort during exercise.

Training changed both what the respiratory muscles could do and how exercise was experienced.

Conclusions
“We found that training specific parameters, such as intensity and number of training sessions, have a much higher impact on improvements in inspiratory training resistance than personal- or lesion characteristics for individuals with SCI (Spinal Cord Injury). We therefore suggest focusing on a high training intensity and perform such a training with about 70 repetitions per session and for 3–5 times a week, over at least several months (>3 months) for inpatients with acute SCI, independent of lesion- or personal characteristics.”

A closer look at IMT in women with COPD.

This randomized controlled trial (Yilmaz et al., 2026) found that four weeks of combined inspiratory muscle training and aerobic exercise improved inspiratory performance, walking distance, dyspnea, and well-being in women with stable GOLD stage I–II COPD.

Several outcomes declined after training stopped, highlighting the importance of continued training and the need for longer-term research.

For generations, voice training has emphasized taking a deep breath. But what if the real key to vocal performance isn’t how you breathe in, but how you control your breath on the way out?

A new review by Jin et al. (2026) challenges one of the most common concepts in voice instruction, suggesting that effective voice production depends less on inspiratory capacity and more on precise expiratory control. The authors highlight that subglottal pressure, the driving force behind vocal intensity and pitch, is regulated through coordinated respiratory muscle activity during exhalation.

For clinicians working with individuals with voice disorders, this perspective is especially interesting. Expiratory Muscle Strength Training (EMST) is designed to strengthen the muscles responsible for generating expiratory pressure, the same system that contributes to cough, swallowing, and the breath support needed for voice. While EMST is not a replacement for traditional voice therapy or singing instruction, this review reinforces the importance of expiratory control as a key component of healthy voice production and highlights an area worthy of continued research.

Not all Expiratory Muscle Strength Training (EMST) is created equal.
A new study explored how changing two key exercise variables, resistance and time under tension (TUT), affects the body’s response during EMST. The researchers found that these training parameters significantly influenced respiratory muscle excursion, submental muscle activation, and perceived effort.

Just as in strength training for other muscle groups, how you train matters. Adjusting the resistance and the length of each expiratory effort may influence the physiological response to EMST and could help optimize training for cough, swallowing, and respiratory function.
As respiratory muscle training continues to evolve, studies like this help move us beyond simply prescribing EMST toward prescribing it more precisely.
