MYTH OR FACT?

When we speak with parents about airway health and myofunctional therapy, one question comes up again and again:

Can myofunctional therapy shrink enlarged tonsils or adenoids?

The answer is no—but here’s why.

Myofunctional therapy is designed to improve oral function—not change anatomy.

It can help improve:
* Proper tongue rest position
* Lip seal
* Swallowing patterns
* Nasal breathing when the airway is open enough for nasal breathing to be possible
* Coordination of the muscles used for breathing, chewing, and swallowing

What it cannot do is reduce the size of enlarged tonsils or adenoids.

If enlarged tonsils or adenoids are significantly blocking a child’s airway, that’s a structural issue that should be evaluated and treated by the appropriate medical provider. Once the airway is open enough for nasal breathing, myofunctional therapy can play an important role in helping children establish healthier breathing patterns and oral function.

This isn’t about choosing medical intervention or avoiding medical intervention.

It’s about identifying why a child is mouth breathing and making sure they receive the right treatment at the right time.

Every child deserves an individualized myofunctional evaluation.

Some children benefit from a collaborative airway team that may include an ENT, an airway-focused dentist or orthodontist, and a myofunctional therapist.

Squeaky Swallow may look simple, but there’s a lot to coordinate at once.

When a client is having difficulty putting all the pieces together, breaking the exercise down can help build the skills needed for the full movement.

For this client, maintaining tongue-to-palate contact while coordinating the repeated lip movement and suction needed for Squeaky Swallow was challenging. During the full exercise, an elastic at the tip of the tongue provides feedback for maintaining tongue-to-palate contact while the lips repeatedly open and close to suction the water into the mouth, followed by a smile swallow while maintaining proper tongue position. 

To break down the skill, we used pudding through a straw with a lip block to limit how much of the straw entered the mouth and encourage lip stabilization and control. The thicker consistency required greater suction and provided more feedback as she practiced repeatedly opening and closing her lips on the straw, while continuing to work on maintaining tongue-to-palate contact.

Once those pieces became more coordinated, we returned to Squeaky Swallow—and she was able to bring the components together, maintaining tongue-to-palate contact while coordinating the lip movement, suction, and smile swallow. 👏

She never sucked her thumb. She gave up her pacifier at 11 months. And yet, by age 5, she needed palatal expansion.

With her family’s permission, we’re sharing part of this sweet girl’s story because it highlights something we wish more parents knew: oral development isn’t only influenced by habits like thumb sucking and pacifier use.

She was a chronic mouth breather and had significant airway obstruction. Before beginning expansion, she had already undergone an adenoidectomy and tonsillectomy.

Why does that matter?

A high, narrow palate can encroach on the floor of the nasal cavity, reducing available space for nasal airflow. Its shape can also make it more difficult for the tongue to maintain a proper resting position against the palate, contributing to a lower, more forward tongue position.

A narrow upper arch can also mean less space for developing adult teeth to erupt.

At age 5, she began palatal expansion with Carter Smile . Her story is a great example of why breathing, airway, tongue position, oral function, and dental development shouldn’t be viewed separately.

For us as myofunctional therapists, how a child breathes and where the tongue rests matter. Chronic mouth breathing can be an important clue that there may be more to investigate.

The goal of myofunctional therapy isn’t just to do the exercises correctly. It’s to make those skills a habit.

By the final phase of therapy, our clients have practiced their exercises consistently—in sessions and at home. Now the focus shifts from learning new skills to making them automatic.

One way we do that is by connecting healthy habits to everyday routines.

This client drew four things she sees or experiences throughout her day. Each one serves as a reminder to pause and check:
👅 Is my tongue resting on the spot?
👄 Are my lips together?
👃 Am I breathing through my nose?

Over time, those intentional check-ins become habits—and that’s exactly how lasting change is created.

We remember what it was like when we were first learning myofunctional therapy, so we would never expect parents to leave a session remembering every detail of their child’s home practice. That’s exactly why we have created short homework videos that families may revisit throughout the week any time a refresher is needed.

Our goal is to make home practice feel a little less overwhelming and a lot more manageable.

Therapy isn’t only about teaching new skills during the session. It’s about supporting families throughout the week.
