IDPH - ILLINOIS DEPT OF PUBLIC HEALTH - they’re so corrupt they’re using Survey to circumvent and collect $800 while they issue License to Assisted Living Facilities to compete with Small Business Agencies that pays $1,500.00 Annual License Fee - the most expensive in any US state compared to East Coast or Florida which is only $50-$300 yearly . Illinois is indeed the most corrupt state !!!
