Hospice medical reviews are increasing. Is your documentation ready?

Hospices are seeing more additional documentation requests (ADRs), with auditors taking a closer look at long patient stays, terminal prognosis and clinical documentation. Longer stays can bring added scrutiny, particularly when documentation does not clearly support continued hospice eligibility. Auditors may also look for evidence that a patient's condition continues to support the terminal prognosis. If a patient improves or stabilizes, Medicare may determine that hospice eligibility is no longer supported. Election statements are also receiving attention, with some denials tied to insufficient information about services waived upon hospice election.

Make sure your team understands where auditors are focusing and how strong documentation can help support hospice eligibility.
