Why don't more surgical residents choose rural practice? The answer is structural, and it starts in training.

→ 85% of graduates subspecialize: only ~15% enter General Surgery practice at all.
→ Residency training focuses on robotics and laparoscopy, not the broad-scope surgery rural hospitals need.
→ Residents train in urban academic centers and rarely rotate rurally.
→ Lower rural compensation and professional isolation further deter rural practice.

Follow the math from residency match to rural practice, and the crisis becomes impossible to ignore.

In 2025, 1,787 General Surgery residency slots were filled nationally. Of those, only ~15-20% enter general practice rather than subspecializing. Of those, just 6.2% choose rural practice.

The result: the U.S. produces roughly 13 to 22 rural-ready General Surgeons per year against an estimated 1,700 open rural General Surgery positions.

This is not a pipeline. It is a trickle against a flood.

Rural America makes up 15% of the U.S. population, but is served by only 10% of the nation's surgeons.

3Dhealth's national study projects demand for surgical specialists will grow 39% over the next 20 years, driven by the aging population. Rural markets will feel that growth most and are the least equipped to absorb it.

3.15 General Surgeons per 100,000 people in small and isolated rural markets. 5.44 per 100,000 in urban markets.
