An open practice is not the same as an accessible one.

Median new-patient wait by specialty, at the point of contact:
→ Pediatrics: 13 days
→ General Surgery: 14 days
→ Cardiac Surgery: 14 days
→ Family Medicine: 15 days
→ Cardiology: 24 days
→ Dermatology: 36 days

In our consumer research, 55% expect a Primary Care appointment within four days. Family Medicine sits at 15, Pediatrics at 13.

Specialists run longer, and some run much longer: Rheumatology at 42 days, Neurology at 44.

Dermatology produces the most office visits here and takes the longest to get into. Cardiac Surgery produces the fewest and is among the fastest. Productivity and access are different measures.

Ask a health system whether its physicians are accepting new patients and you’ll get an answer. Ask the physicians and you’ll get a different one. Call the practice the way a patient would and you’ll get the truth.

118,000+ calls testing physician availability, across 42 states and 16 years so far. Not surveys. Calls.

Primary care practices are closed to new patients 24.1% of the time. Specialists, 6.8%. More than three and a half times the rate, in the half of the network most patients enter through.

Primary care is the front door. When it’s closed, the rest of the network doesn’t exist for that patient.

Then the part that should sting. Where a patient couldn’t be seen and an alternative existed, one was offered 6.6% of the time. The other 93.4% got no advanced practice provider, no partner site, no next step.

That’s why we verify instead of asking.

Headcount answers a real question, and it is the first one we answer for every client. How many physicians does this population need, by specialty? Nothing replaces that analysis.

Ron's session is about the question that follows. What does each of those physicians actually deliver in patient access, and how do you measure it? It is the same question we are working through on this page over the next few weeks.

Before we measure patient access, look at what one clinical FTE produces.

Median office visits per year, five-year MGMA average. These are the capacity assumptions our own demand model runs on:

→ Dermatology: 5,321
→ Pediatrics: 3,548
→ Family Medicine: 3,265
→ Cardiology: 3,049
→ General Surgery: 1,730
→ Cardiac Surgery: 819

Same unit. A 6.5x spread.

Cardiac surgeons aren't underperforming. Their work is in the OR, not the office, which is exactly why office visits alone can't tell you what a surgeon delivers.

And visits aren't access anyway. A physician logging 5,000 visits a year might see 1,250 patients four times each, or 2,500 patients twice. Same visit count. Twice the people.

Visit volume is the starting point, not the finding. Later in the series we convert it into Served Lives: the patients behind the visits.

Most health systems plan their physician workforce in FTEs. So do we. Every Provider Development Plan we build uses the FTE as a measure of clinical time.

But a Primary Care physician at the 25th percentile of MGMA productivity serves about 990 patients. At the 75th, about 1,700. Same specialty, same 1.0 clinical FTE.

The FTE measures time. It doesn't measure what that time produces.

So we measure patient access three ways:

→ Verified Access: is the practice open to new patients, confirmed by calling rather than asking.
→ Access Velocity: how long a new patient waits.
→ Served Lives: how many patients that FTE actually manages.

Five weeks, one at a time, with our own data. We call the gap between clinical time and patient access the FTE Fallacy.

Over the last six weeks, we've shared what the data reveals about rural General Surgery in America.

The rural General Surgery crisis is not one problem. It is five simultaneous failures.

1. A pipeline producing 13-22 rural surgeons per year against 1,700 open positions.
2. A workforce already operating at or above capacity.
3. A retirement wave removing 1.5 FTEs of access per surgeon lost.
4. Restricted access already hitting 1 in 4 practices and rising.
5. Consumers who won't wait and increasingly have nowhere else to go.

No single policy fixes all five. But understanding all five is where honest planning has to start.
