📏 Planning a DFO? Your technique choice changes leg length — and now you can predict exactly how much.

In a two-center study (Rush + Mayo Clinic), we validated a simple radiograph-based formula that predicts leg length change after valgus-correcting distal femoral osteotomy:

🔵 Lateral opening-wedge (LOW) → ~0.85 mm of LENGTHENING per 1° of correction
⚪ Medial closing-wedge (MCW) → ~0.45 mm of SHORTENING per 1° of correction

At a 10° correction, that’s roughly +5.9 mm (LOW) vs −4.5 mm (MCW) — a difference that matters in patients with preexisting leg length discrepancy.

The prediction tool? Spot on. Predicted change was statistically equivalent to the true change measured on postoperative radiographs (LOW P = .16; MCW P = .85) — all from standard standing full-length films, no special imaging needed.

The takeaway for planning: screen for leg length discrepancy before DFO. If the operative leg is already longer and a large correction is planned, MCW may be the better choice; if it’s shorter, LOW works in your favor. Patient-specific calculation gives the most accurate prediction.

🧊 Does keeping a graft cold during harvest actually matter? Our latest study says yes — a lot.

Thermal injury during osteochondral allograft (OCA) reaming can kill the chondrocytes you’re counting on for long-term graft survival. We compared three harvest techniques across 10 fresh human femoral condyles:

🔴 Dry harvest → peak internal temp 34.8°C, viability 58.7%
🟡 Handheld cold saline (bulb) → 16.9°C, 64.6%
🔵 Cold saline submersion → 8.9°C, 75.2% — on par with native cartilage

The takeaway for the OR: submerging the graft in cold saline during reaming cut peak temperatures roughly 4-fold and preserved chondrocyte viability better than dry or handheld irrigation. It’s a simple, reproducible step that may improve long-term graft durability.
