Northwest Health Services is a community health center with locations spread over 15 counties. Community health centers are local, non-profit, community-owned health care providers serving low income and medically underserved communities. Also known as Federally Qualified Health Centers (FQHCs), they are located in areas where care is needed but scarce, and improve access to care for millions of Americans regardless of their insurance status.

Community Health Centers are on the front lines of preventing and managing chronic disease.
In 2025 alone:
✅ 3.7M patients received diabetes care — 73.6% reached A1c control
✅ 6.4M were treated for hypertension — 68.9% achieved blood pressure control
✅ 10.5M received obesity care — 74.6% of youth got screened and counseled on nutrition & activity
