Family Medical Clinic of Chubbuck is transitioning to Matix Health to bring our general practice and specialty services together under one brand that better reflects our mission, vision, and future.

While our name and look are changing, our ownership, providers, staff, and commitment to our patients are not. You can continue to expect the same people, the same personalized care, and the same commitment to serving our community.

We’re excited for this new chapter and can’t wait to share what’s ahead!
