My dose went up this week. We waited 28 days on purpose.

Month 2 of my experiment. I'm on the same GLP-1 protocol my patients follow, documented on camera, no filters.

Here's what most people get wrong about dose increases. They think faster means better results. It doesn't. It means more nausea, more quitting, more "this didn't work for me" posts.

The protocol is: start low. Hold 4 full weeks. Then increase based on 3 things. Your tolerance. Your labs. Your results. Not your impatience.

Creatine. The most studied supplement on earth. 500+ human trials. And it got branded as a powder for 22-year-olds who scream at barbells.

Here's who it's for in my pharmacy: anyone losing weight.

When you're on GLP-1 therapy, you eat less. Your muscle pays the bill unless you defend it. My 3-part defense for every weight loss patient:

Resistance training 3x a week. Non-negotiable.
150g+ of protein a day.
5g of creatine daily.

I take it every morning. It's in my own protocol. And it's on my shelf for a reason.

One more myth to retire: creatine doesn't damage healthy kidneys. The research on this is boring, which is the best thing research can be.

My bedtime injection has 1 rule: no carbs for 2 hours before.

Part of my nightly protocol is a growth hormone peptide. It doesn't give you growth hormone. It signals your pituitary to release your own.

Here's why the timing matters. Your biggest natural growth hormone pulse happens in deep sleep. That pulse is what repairs muscle, burns fat overnight, and deepens sleep quality.

Insulin blunts that pulse. Eat carbs at 8pm, inject at 9, and you worked against yourself.

It's 100 degrees in Cibolo and your medication turned down your thirst.

Here's what almost nobody tells you about GLP-1 therapy. It quiets appetite signals. It quiets thirst signals too. You stop feeling hungry and you stop feeling thirsty, in a Texas August.

Nobody posts about the bathroom part.

Everyone warns you about nausea on GLP-1 therapy. Almost nobody warns you about weeks 2 through 6, when things stop moving.

Here's why. GLP-1 therapy slows your digestion. That's the mechanism. It's how your appetite gets quiet. But a slower gut means slower everything.

What I tell my patients:

Get 25g of fiber a day. Psyllium counts.
Drink 80+ oz of water. Fiber without water makes it worse.
Walk 10 minutes after meals.
Magnesium glycinate at night helps most people, and it helps your sleep too.

If nothing moves for 3+ days, call your pharmacist. Not Google.

Managing side effects is the difference between quitting in week 3 and reaching your goal. That's why every patient I work with gets a side-effect plan from day 1, coordinated with their prescriber.

2 drinks hit like 4 on GLP-1 therapy.

Nobody warns you about this one. GLP-1 therapy slows your stomach, so alcohol absorbs differently and blood sugar swings harder. My patients report the same 3 things:

1. Drinks hit faster and stronger than they used to.
2. Hangovers got meaner.
3. The desire to drink faded. Some lost interest completely.

That third one surprises people. The same signals that quiet food noise can quiet alcohol noise too. Researchers are studying exactly that right now.

My advice if you drink on therapy: go slower than your old normal, eat something first, and double your water. And if the craving fell away on its own, let it go. That's not a side effect. That's a gift.

A hot car can wreck your medication in 1 afternoon.

It's summer in Texas. Your dashboard hits 140 degrees. Injectable therapies aren't built for that, and every summer I see months of progress put at risk because medication rode in a trunk to the coast.

My 4 travel rules:

1. Insulated cooler bag with an ice pack. Medication in the middle, never touching the ice. Frozen is as bad as cooked.
2. Flying? Carry it on. Checked bags freeze at altitude and bake on the tarmac. TSA allows medication with ice packs. Keep it in original packaging with the label.
3. Hotel mini fridge the moment you check in. Not the car. Not the beach bag.
4. Keep your injection day, even on vacation. The schedule is part of the treatment.

5 minutes of packing protects months of progress.
