📣 Looking for an Adult Day Program? We Have Open Enrollment!
Transitions BH Adult Day is currently accepting new participants at our 5041 Oaklawn Dr. location.
Wondering how enrollment works? Adults interested in waiver-funded Adult Day services can start by talking with their County Board of Developmental Disabilities Service and Support Administrator (SSA) about eligibility, available services, and whether Transitions BH may be a good option for their individualized service plan.
🚌 Convenient access matters, too. Our Adult Day location is accessible along the MetroAccess bus line, helping support transportation and community access.
Our focus goes beyond simply having somewhere to go during the day. We want participants to build skills, relationships, independence, community connections, and meaningful routines based on what matters to them.
