Before discharge day, take a fresh look around the house.

A few things families may want to check:

✓ Are frequently used walkways free of clutter and cords?
✓ Is there good lighting in hallways, bathrooms and near stairs?
✓ Are loose rugs creating a potential trip hazard?
✓ Has the bathroom setup been reviewed?
✓ Is there a plan for meals, dressing, transportation and everyday routines?
✓ Who will be there when family members are at work?

For individual fall-risk concerns or recommendations about equipment or home modifications, talk with your loved one’s physician, physical therapist, occupational therapist or other healthcare professional.

And remember the physical house is only part of the plan.
