Step-by-Step VF/Pulseless V-Tach Arrest Management

1💎Confirm cardiac arrest — check pulse (≤10 seconds).

2💎If pulseless, begin high-quality CPR immediately

3🫀Attach defibrillator/monitor and analyze rhythm. VF is a shockable rhythm.

4🤲Resume CPR immediately for 2 minutes after shock 

5💎 Establish IV/IO access during CPR (IV preferred over IO) & Begin addressing reversible causes (H's and T's). 

6🫀Rhythm check at 2 minutes — if still VF/pVT:
Shock #2 (same or escalating energy)

7🤲 Resume CPR immediately

8💉Epinephrine 1 mg IV/IO (give as soon as feasible after 2nd shock; repeat every 3–5 minutes) 

9🫀Rhythm check at 2 minutes — if still VF/pVT:
Shock #3

10💎Resume CPR immediately

11💉Antiarrhythmic: either amiodarone or lidocaine: 

12🤲Continue 2-minute CPR cycles — repeat rhythm checks, shocks if still shockable, and epinephrine every 3–5 minutes. A second dose of antiarrhythmic may be given if VF persists.

💎💎24-48 hours post-surgery: Suctioning every 4 hours (and as needed)

💎💎💎Beyond 48 hours: Frequency adjusted based on secretion production, typically every 2-4 hours.

The inner cannula should be removed twice daily for cleaning, and patients require airway inspection around the clock, including during sleep.

Protocol adherence and staff training are critical.
