A proposed CMS rule (CMS-2449-P) would cap Medicaid supplemental payments — including GEMT — at Medicare Ambulance Fee Schedule rates starting in 2029, with phase-downs beginning in 2028. For public and rural EMS agencies that depend on GEMT funding to cover the real cost of a 911 response, this is a threat to operational readiness.
