We have a number of openings & are looking to grow our team in Latham, NY!
As a Care Manager, you’ll play a vital role in coordinating services, advocating for children’s & adult's needs, and ensuring they receive the best possible care and support. This role is perfect for someone who is compassionate, organized, and dedicated to improving the well-being of children, adults and their families.
Key Responsibilities:
✅ Develop and implement individualized care plans
✅ Collaborate with families, schools, and healthcare providers
✅ Connect clients with essential resources and services
✅ Monitor progress and adjust plans as needed
✅ Advocate for client's health, education, and social needs
Qualifications:
✔ Bachelors Degree in social work, psychology, or a related field (Associates Degree with experience will be considered)
✔ Experience working with children, adults and families
✔ Strong communication and problem-solving skills
✔ Ability to work independently and as part of a team
