Diamond Willow Assisted Living: Cloquet We offer small intimate community settings of 8-16 residents per home and site based RN’s who work With that comes the belief that bigger is not always better.

Here at Diamond Willow we take pride in our person centered care model which is uniquely different from most diagnosis based care systems. Each setting is designed to be intimate with fewer residents per suite, allowing us to have smaller communities to promote our person centered care model in a truly calm and comforting home-like environment. This affords us the ability to provide one on one att

ention in a more positive, healthy atmosphere to each and every individual. It is because of this model that we offer the industry leading staffing ratios of 1:5 during awake hours and 1:13 overnight. We also recognize that moving multiple times later in life is everything short of desirable. As care needs increase and evolve our person oriented program is designed to provide varying levels of care in a personal suite from arrival through end of life, highly reducing the need to change facilities in the future.

It was wonderful to see so many families and loved ones come together to spend time with our residents and make such special memories.

A special shoutout to Gentiva Hospice for bringing the fun with bubbles and tattoos, and to Brad for providing the music and keeping the celebration going! 🎶🫧

There were lots of laughs and plenty of friendly competition! Residents had a great time tossing balloons, and somehow our staff ended up being the favorite targets. Between the balloons and the pool noodles, everyone got a good laugh! 🙂
