Cokato Charitable Trust began with a simple question: how can we make sure older adults in our community can receive quality care close to home?

That question brought local leaders, community members, and area congregations together in the early 1960s, laying the foundation for the care and services CCT provides today. Since then, our organization has continued to grow and adapt to meet the changing needs of older adults and families, while remaining locally governed and deeply connected to the community we serve.

Today, that commitment continues across independent living, assisted living, skilled nursing, rehabilitation, therapy, and community services. Our mission remains the same: to serve older adults with dignity, compassion, and respect.

Recovery looks different for everyone.

Cokato Charitable Trust provides rehabilitation services onsite through Pro Rehab for residents and members of our surrounding community.

Physical, occupational, and speech therapy are available for individuals of all ages recovering from surgery, injury, illness, or changes in balance, mobility, speech, or swallowing. Each therapy plan begins with an evaluation and is built around individual needs and goals.

Outpatient therapy is open to community members with a physician referral, so you don't have to live at one of our communities to receive care.

A look back at one of Cokato’s favorite summer traditions!

Our residents and staff enjoyed getting into the spirit of Cokato Corn Carnival with some corn shucking, festive activities, and a special evening celebrating the community. And, of course, parade night was a highlight!

We’re excited to share a few photos from Corn Carnival and revisit these wonderful community memories with you.

Thank you to everyone who makes Cokato Corn Carnival such a special part of our community. We’re grateful for the opportunity to celebrate alongside our residents, families, staff, and neighbors!
