✅ Step 1: Start with your primary care physician.
Ask them for a home health order and referral. Your doctor knows your medical history best and can quickly get the paperwork started.
✅ Step 2: You must meet two key requirements:
• Have a skilled need — care that requires a licensed professional, such as: •Medication management or teaching •Wound care •Chronic disease management (diabetes, heart failure, COPD, and more)
• Be homebound — meaning it’s difficult for you to leave home without considerable effort, assistance, or the help of a device (like a walker or wheelchair). Leaving home is infrequent, short, or requires taxing effort. (Physical therapy works the same way!)
