We understand you are in open-enrollment right now and there is some confusion about plans with BSW and what we accept. If you are our patient, you will want the ACCESS plan in order to remain our patient. Per our credentialling department at our billing company, that is the plan that we are IN NETWORK with. We want you all to have the best coverage with our office, so hoping this will help clear things along for you all!

There is quite a bit of confusion on what exactly is an ANNUAL WELLNESS VISIT. By definition:
An Annual Wellness Visit (AWV) is a yearly preventive care appointment with your primary care provider designed to create or update a personalized prevention plan. It is not a head-to-toe physical, but rather a comprehensive review of your medical history, current medications, and risk factors.

What to expect during the appointment your healthcare provider will generally:
Review your history: Discuss your personal and family medical/surgical history.
Assess vitals: Check your height, weight, body mass index (BMI), and blood pressure.
Health Risk Assessment: Fill out a questionnaire regarding your overall health, diet, physical activity, and emotional wellbeing (including screenings for depression and cognitive impairment).
Screening Schedule: Create a 5-to-10-year personalized checklist for recommended, age-appropriate preventive services and immunizations.
Advance Care Planning: Discuss your future healthcare preferences or advance directives in case you are ever unable to make your own medical decisions.

At the discretion of your provider, anything outside of this information discussed during the visit may result in a copay from your insurance.

Thank you very much for your understanding when you receive a bill for a copay from your annual wellness visit. It is just our office trying to be thorough and maximize your time, rather than requiring you to return to our clinic for another visit for your chronic or new conditions. We want to be as thorough as possible in order to give you the best possible medical care and consideration.
