You’ve heard that disc herniations are often harmless in that they exist very commonly without causing any pain. (PMID 26359154).

But even when they do cause pain, they very often heal on their own. And I’ve got the receipts.

Sometimes, such as when there are emergent red flag symptoms, or when symptoms do not respond to good conservative care, surgery can be indicated.

But it’s definitely not every time. In fact, the majority of the time, they do not require surgery.

Every person’s situation is unique. But I am just highlighting for you what the science shows us about how resilient your body is. And this is what I see happen with my patients regularly.

Acute Back Pain resolves on its own within 4-6 weeks most of the time.

Chronic Back Pain can go on for years.

The 2 biggest mistakes I see with chronic back pain are:

1. Not recognizing the role of the nervous system while hyper focusing on tissue damage that is often completely unrelated to the pain
2. Only avoiding and protecting, and never confidently challenging.

It is essential to establish safety first. Once we know you are safe, we can confidently confront and challenge you again to start the road to recovery.

You’ve probably heard this advice before: “You should stick to low impact exercise to protect your joints”.

It’s common medical advice, and honestly probably what most people think “physical therapy exercises” are.

And this advice makes sense in certain scenarios like if you’ve just broken a bone, are recovering from surgery, are having a flare up of osteoarthritis, or are dealing with acute overuse related pain.

But the problem is when this becomes long term advice.

Sticking to low impact exercise, “protecting” your joints, and avoiding stress to them long term will only decrease the capacity of not just your joints, but your muscles, tendons and entire system.

Long term, the best advice for joint health is actually the opposite. Challenging your body in healthy doses is what keeps your joints, muscles, tendons, and really your whole body healthy.

Protection is good as a short term strategy.

As a long term strategy it’s sh*te.

I’m not saying over do it. I’m not saying to ignore pain completely and push through recklessly.

But I am saying that at some point, find a way to challenge yourself in an amount your body will receive positively and adapt to.

That is what really keeps your joints and your whole body healthy.
