🖤 Today, Nikita Breaks Her Silence. 🖤

Years ago, I spoke with a young woman named Arika Trim as she explored her options for support during pregnancy during a consultation.

I never heard back from her.

A week after giving birth, Arika was gone.

What makes her story even more heartbreaking is that so few people know her name.

Born on August 12, 1990, in Tobago, Arika was brilliant, accomplished, and deeply committed to public service. Throughout her career, she served in the Office of First Lady Michelle Obama, the U.S. Department of Agriculture, the U.S. House of Representatives Committee on Education and Labor, and the American Hospital Association.

She was recognized as an Ignite Caribbean 30 Under 30 Honoree and was featured on the cover of African American Career World Magazine.

She was a daughter.
She was a leader.
She was a mother.

And she should still be here.

Arika's story reminds us that childbirth is not the finish line. The postpartum period matters. Listening to mothers matters. Taking concerns seriously matters.

Behind every maternal mortality statistic is a woman whose life, dreams, and family mattered.

Today, we remember Arika Trim and honor her legacy by continuing the fight for better maternal health outcomes for Black women.

🩺 HIGH-RISK BIRTHS OFTEN REQUIRE HIGHER LEVELS OF SUPPORT.

One of the biggest mistakes a doula can make is believing that every intervention is unnecessary.

The reality is that some pregnancies and births become medically complicated and may require interventions to protect the life and health of the mother, baby, or both.

This may include:

✔ Induction of labor
✔ Continuous fetal monitoring
✔ Blood pressure medications
✔ Magnesium Sulfate
✔ Hospital transfer
✔ Cesarean birth

As doulas, our role is not to stand in the way of medically necessary care.

Our role is not to persuade clients to refuse treatment.

Our role is to support informed decision-making by helping clients understand the risks, benefits, alternatives, and recommendations provided by their healthcare team.

A professional doula understands that advocacy is not about avoiding interventions at all costs.

🚨 STOP CALLING YOURSELF A LACTATION CONSULTANT 🚨

Simply completing a basic breastfeeding class does NOT make someone a Lactation Consultant.

As doulas, childbirth educators, and birth workers, we have a responsibility to accurately represent our training and credentials.

Breastfeeding support goes far beyond teaching feeding positions and discussing latch basics. True lactation support often requires clinical assessment skills, advanced education, and the ability to recognize when a family needs a higher level of care.

A basic breastfeeding certificate does not prepare someone to assess:

• Poor weight gain
• Tongue and lip ties
• Milk transfer issues
• Complex latch problems
• Low milk supply
• Breastfeeding complications
• Maternal or infant medical concerns

Families deserve support from professionals who understand the difference between basic breastfeeding education and clinical lactation care.

As doulas, our role is to support normal breastfeeding, provide evidence-based education, encourage families, and make appropriate referrals when concerns arise.

There is absolutely nothing wrong with saying:

✔️ "I completed a breastfeeding course."
✔️ "I provide basic breastfeeding support."
✔️ "I refer to an IBCLC when concerns arise."

There IS something wrong with overstating your qualifications.

Professional integrity matters.
Patient safety matters.
Families matter.

Know your scope. Honor your training. Refer when appropriate.

🖤 THE HISTORY STILL MATTERS 🖤

When we talk about breastfeeding disparities among Black mothers today, we cannot ignore the impact of history.

During slavery, many Black women were forced to serve as wet nurses, breastfeeding the children of enslavers while their own babies were often separated from them or denied the same access to their mother's milk and care.

For generations, Black women's bodies were controlled, their reproductive choices were taken away, and breastfeeding became associated with exploitation rather than empowerment.

The effects of that history did not disappear after emancipation.

Over time, many Black communities experienced:
▪️ Generational trauma surrounding breastfeeding
▪️ Lack of culturally competent lactation support
▪️ Lower rates of breastfeeding education
▪️ Workplace barriers
▪️ Healthcare bias and discrimination
▪️ Limited representation among lactation professionals

Today, Black mothers are often less likely to initiate breastfeeding and more likely to stop breastfeeding earlier than other groups—not because they care less about their babies, but because systemic barriers continue to exist.

The good news?

Black breastfeeding rates are improving.

More Black lactation consultants, doulas, midwives, peer counselors, and maternal health advocates are working every day to ensure Black families receive the support, education, and resources they deserve.

Breastfeeding is not just a feeding method.

For many families, it is an act of healing, reclaiming knowledge, restoring traditions, and exercising the autonomy that our ancestors were denied.

As birth workers, we have a responsibility to understand the history, acknowledge the disparities, and actively work toward equitable breastfeeding support for all families.
