One of our clients referred someone to us who had asked her to manage her wound care. Our client's response? "No. I'm not doing wound changes for you."

The woman assumed that because our client had once done dressings for her own daughter, she could do them for anyone. But our client knew better. There was a point when her daughter's wound care became too complicated for a family member to handle. It was at that point that we stepped in.

So I started seeing this 85-year-old woman daily for an infected hip replacement wound. It was a serious situation that had already required two rounds of antibiotics. Her doctor sent her home, knowing she needed daily professional wound care, but never mentioned home health resources or referred her to a wound clinic.

Here's the problem: This woman thought anyone could change her dressing. She kept looking for a companion caregiver or home aide to handle it. She was looking for someone who could "just slap a Band-Aid on it."

But this wasn't a band-aid situation. The wound had brown drainage and clear signs of infection that required professional assessment. Every day I went to see her, he was evaluating whether the wound was healing or deteriorating. He was using clinical judgment that a home aide or companion caregiver simply doesn't have the training to make.

This is like calling a carpet cleaning company because your toilet is overflowing, but never once calling a plumber.
The plumber needs to assess why the toilet is overflowing in the first place. But you keep calling the carpet cleaner to mop up the mess. You're treating the symptom, not the problem.

We don't teach our companion caregivers wound care. It's not within their expertise or scope of practice. It's not for them. And I wasn't about to teach an unlicensed person how to manage an advanced wound that could easily become life-threatening.

The healthcare system often asks: How can we make this cheaper? How can we squeeze more visits into less time?

We ask different questions: What does this person actually need? What level of expertise does this situation require? What's the right care, not just the convenient care?

Sometimes you need someone who can cook meals and provide companionship. Sometimes you need someone who can assess whether drainage means your wound is infected and heading toward sepsis.

Knowing the difference isn't just about credentials; it could save lives.

We're not afraid to say when something is beyond companion care. We're not afraid to tell you the truth about what your situation actually requires. Because we'd rather be honest than convenient.

Nobody wants to be the one who brings it up. But waiting until there's a crisis makes everything harder, for them and for you.

Here are the signs it's time to start talking about what's next:

💊They're missing medications or appointments. If you're finding unopened pill bottles or hearing about no-shows at the doctor, their ability to manage their own care is slipping.

🏠The house tells a different story. Spoiled food in the fridge, unopened mail piling up, or a usually tidy home becoming cluttered are all red flags that daily tasks are becoming overwhelming.

🚗Driving is becoming dangerous. New dents in the car, getting lost on familiar routes, or family members refusing to ride with them are serious warning signs.

📞They're isolating more. Skipping activities they used to love, not returning calls, or seeming withdrawn can signal depression or cognitive decline.

😟You're constantly worried. If you're lying awake at night wondering if they're okay, that's your gut telling you something needs to change.

Here's the truth: The best time to have these conversations is NOW, before emotions are running high and options are limited. When it's still hypothetical, you can talk about preferences, values, and plans without the pressure of an immediate decision.

The most dangerous moment can be going home from the hospital.

When we met this couple, the wife was exhausted from years of caring for her husband, who has dementia. She needed help desperately. But he wasn't ready for strangers in his home.

Here's what people with dementia experience when new caregivers show up: Their nervous system screams, "I don't know you. You're not safe." If we'd rushed in trying to help him immediately, we would have made everything worse.
So we did something different. We cared for her first.

We took tasks off her plate—meal prep, appointment coordination, and medication management. We gave her space to breathe. And while we were caring for her, he was watching. Getting used to our faces. Hearing our voices. Seeing us interact gently with his wife.

We left him alone. We didn't force interaction. We didn't try to prove ourselves.

After a few months, something shifted. He started making eye contact. Responding to greetings. Letting us help with small things. He invited us in.

Now we care for both of them—and he feels safe with us because we never rushed him.

This is dementia care done right: Support the caregiver. Give the person with dementia time to trust you. Create stability instead of disruption. Keep them home where their nervous system can stay calm.
