Things I wish every patient knew before starting psychiatric medication — but most prescribers don’t have time to explain. The first medication isn’t usually the final one. You may feel worse before you feel better. Side effects often fade. You’ll wonder if it’s changing who you are. And medication works best when it’s not the only thing you’re doing. If your prescriber didn’t tell you any of this, you deserve better care. Ohio patients — link in bio.

Hi, I’m David Glenn psychiatric nurse practitioner and owner of Real Psychiatric Services, an outpatient practice serving adolescents and adults across Ohio. I diagnose and treat depression, anxiety, ADHD, bipolar disorder, OCD, PTSD, insomnia, and substance use disorders. My appointments aren’t 15-minute med checks — they’re real conversations about what’s actually going on. I take a whole-person approach: sleep, labs, hormones, stressors, the full picture. In-person and telehealth available anywhere in Ohio. New patient appointments open — link in bio.

Why are so many adults finding out they have OCD in their 30s and 40s — after a lifetime of being told it was just anxiety? It’s not a coincidence. For decades, OCD was portrayed as hand-washing and checking, so the mental kind — intrusive thoughts, rumination, mental rituals — went unrecognized. Many of you developed perfectionism and over-preparation as coping strategies, and they worked until life got harder. The good news: adult-onset treatment works. ERP therapy + the right medication strategy can change your life. If you’re in Ohio, link in bio.

How do you tell if your “overthinking” is actually OCD? The treatment is completely different — and most people walk around for years misdiagnosed with generalized anxiety. The 4 signs to watch for: ego-dystonic thoughts that horrify you, mental rituals, relief that never lasts, and standard anxiety treatment that hasn’t really helped. If this sounds like you, it’s worth bringing up with a psychiatric provider specifically. 👉 Follow for Part 13 on why so many adults are discovering OCD in their 30s and 40s.

What do I actually prescribe for patients stuck in thought loops? It’s probably not what you think. Before reaching for medication, I’m checking 4 things first: sleep, OCD vs rumination, undiagnosed ADHD, and whether therapy is in the picture. Most “treatment-resistant” rumination isn’t resistant — it’s just been treated incompletely. 👉 Follow for Part 12 on how to tell when overthinking is actually OCD.

Why is overthinking always worse at 3am? It’s not your imagination. Your prefrontal cortex is offline, cortisol is rising, and your brain has zero distractions — so every thought hits at full volume. Here’s what’s actually happening and the one shift that helps. 👉 Follow for Part 11 on what I prescribe for thought loops. Comment what time your worst overthinking hits.

Why does your brain replay embarrassing moments from years ago? It’s called retrospective rumination, and it’s incredibly common in anxiety, depression, ADHD, and trauma. Here’s why your brain keeps reopening that file — and the one shift that actually closes it. 👉 Follow for Part 10 on why 3am rumination is different. Comment which decade your brain keeps dragging you back to.

Not all self-reflection is healthy.

A lot of people think they’re being self-aware… when they’re actually trapped in rumination.

Healthy self-reflection helps you understand yourself and move forward.

Rumination keeps you stuck endlessly analyzing yourself:
👉 “Why am I like this?”
👉 “What’s wrong with me?”
👉 “Why do I keep feeling this way?”

The problem is that when you’re emotionally distressed, your thinking becomes less objective.

So instead of gaining clarity, you often spiral deeper into anxiety, shame, confusion, or self-criticism.

Self-awareness should lead to understanding and action — not endless self-analysis.

You do not need to completely “figure yourself out” before you can start living your life.

When you catch yourself spiraling inward, ask:
👉 “Is this reflection helping me move forward… or just keeping me mentally stuck?”

That question alone can shift the pattern.

If your brain constantly asks ‘what if?’… it’s probably trying to protect you.

A lot of rumination comes from the mind trying to predict and control the future.

👉 “What if I made the wrong decision?”
👉 “What if this goes badly?”
👉 “What if something bad happens?”

The brain starts believing that if it can think through every possible outcome, it can finally feel safe.

But the problem is:
The mind begins treating uncertainty like danger.

So instead of accepting uncertainty as part of life, it tries to eliminate it completely—which is impossible.

Peace does not come from predicting everything perfectly.

It comes from learning that you can tolerate uncertainty and still handle life.

When you catch yourself spiraling into “what ifs,” ask yourself:
👉 “Am I preparing for reality… or chasing certainty that doesn’t exist?”

That question alone can interrupt the loop.

If you constantly need reassurance to calm your thoughts… this might be why the overthinking keeps coming back.

A lot of people deal with rumination by seeking reassurance:

👉 “Do you think everything is okay?”
👉 “Do you think they’re mad at me?”
👉 “What if this means something bad?”

And reassurance does help temporarily.

But over time, your brain starts learning:
👉 “I can’t handle uncertainty unless someone reassures me.”

So the next time anxiety shows up, the cycle starts all over again.

Reassurance relieves anxiety short-term…
but often strengthens it long-term.

The goal isn’t to become 100% certain about everything.

The goal is learning that you can tolerate uncertainty without feeding the loop.

That’s where real freedom starts.
