As the summer is coming to an end, I am reflecting on the fact that I have been treating Craniofacial Pain that is more often called "TMJ" (which is the initials for the TemporoMandibular Joint not the Disorder itself) for nearly 20 years now. Wow!!! That is hard for even me to believe.
I myself was a patient in the TMJ and Sleep Therapy Centre of San Diego that long ago. I was miserable for so many years before I found out through that Centre what was actually going on with me. Strange to think since I am a dentist huh!!!. My life was changed so drastically that I decided to be educated by them and limit my practice becoming a Centre myself. This disorder is still so misunderstood by the medical and dental community. My training cleared up that misunderstanding for me.
This week I evaluated a patient who has been in treatment elsewhere with another provider for quite some time(and still is in treatment there technically). She has not gotten any relief! The treatment that she was being given reminded me that the disorder is still very misunderstood in dentistry.
My greatest joy comes from seeing my patients transform into the person that they really are instead of the person that the disorder has forced them to be. I have been so very blessed to see so many go through this transformation!

If you don't believe that poor facial growth and development begins early in childhood, then I am sorry that you don't. These photos are the results in adulthood of the poor growth and development that begins early in childhood. I am not just looking at facial beauty. It is easy to see signs of poor health for certain. I see it everyday in the adult patients that I treat with pain. Mouth breathing is rampant in my patient population. One of the photos of the children was treated by me as she is my daughter. I promised myself that she would not have the retractive orthodontic nightmare that I suffered. I went down the education path that I chose much due to her!
Her teeth were not coming in when and as they should so we started early with her treatment. The first photo is her at a young age where her teeth had no room to erupt. Extraction of teeth? NO WAY!!! Not under my watch. I suffered much of my adult years from about 23 years old to 45 years old after retractive orthodontics sent me into a pain pattern that was torturous!! Not to mention how it messed up my airway and my face eventually resulting in seizures. NO WAY was I going to ever allow my child to suffer this way. She did unfortunately have airway problems because that came on very early as it does now in almost all children. Statistics support this. Children are being diagnosed with sleep apnea for heaven's sake!!!
The second photo was not just cosmetic changes. We improved her airway at the first level. The second photo is the result. We have now improved her airway at the second level. Of course I am a bit partial since she is my daughter, but I would say she received facial beauty to a degree that most dream of because we established an airway thus her health!! She is now a Registered Dental Hygienist.

The "old" assumption that facial growth and crooked teeth are totally genetically determined is a myth of the past. Yes, we do inherit the likelihood for features of course, but the actual growth and development depends on something totally different.

I want to show some photos of children that we look at as "sooooo cute", but in reality they are poor development that will lead to poor health in the future. These children all are experiencing poor growth and development due to mouth breathing. These poor growth patterns can be changed. The younger the better!!!

Thumbsucking and pacifier use is a sign of a poorly developing airway that will alter a child's growth and development bringing long lasting problems. Problems such as worsened airway problems and TMJ Disorder due to that poor growth and development.

Rebecca came into our office with her mother and daughter having to help her walk in. She could barely speak to me at that first appointment. We did a full evaluation. Determined exactly what was the problem. She started on an immediate "temporary" phase of care to get her to a place where we could start her full treatment. The first photo depicts how she was on the "symptom treatment" that was the only way the medical community knew to treat what they saw going on with her. She came back the next week to start her full treatment which is depicted in the second photo. Just when I thought that I couldn't be surprised anymore...this did it!!!

Something I have learned over the nearly 20 years of treating patients for the disorders that I treat (30 years of dentistry in 2022) is that not every patient is open to or prepared to hear the information about the reality of their problem. They may have a medical background that keeps the information I am giving them from being comprehended by them. Or even worse they may criticize me for the knowledge (professional and personal experience) that I have. I co-treat with medical professionals, and I have found out that they don't always understand either. When I educate them and they see the patients improve so much, they are amazed. They may not fully "get it" but they are very happy to see their patients getting so much better!
The fact that I suffered for 20+ years with pain (I went through Dental School in that level of pain). It was becoming almost debilitating which puts me in a place of "understanding" of where my patients are. My life and health have been completely changed for the better after I was treated. The sad part about a patient not wanting to hear the truth about their problem means that they will be suffering for some time to come, or they will have symptom treatment not resolution.
I just spoke to a patient who told me about her friend who was telling her about this friend having a migraine. My patient said to me "hummm, I haven't had a migraine in so long I can almost not remember what it was like." Yeah!!!!! I totally understand.
I joke with my patients that if I had even a slight headache that I would think that I had a brain tumor or something because it has been 15 years since I have had a headache of any kind. Headaches were all I had before my treatment.
My care of my patients comes from a compassion that comes from a total understanding of their situation. I also treat children so that they never get to that place.

At our Centre, we treat children. The problems start early. They may not be obvious to parents. Children don't complain about pain very often because at that stage of life they don't know that pain isn't normal. They think everyone feels that way. They may not sleep well. They have crowded teeth (even baby teeth). Crowded teeth are a sign of poor development that comes from poor upper airway function not genetics. They may be labelled ADD or ADHD or even worse they may be labelled with a low IQ. Snoring is a definite sign. Yes, children can have sleep apnea, but it must be treated very differently than it is traditionally treated in adults. It is a vicious cycle. Airway problems affect their facial growth which in turn is even worse for their airway. This is why there are so many adults with sleep apnea on a CPAP. These adults will always develop other health problems such as Type II Diabetes and High Blood Pressure also. We strive to keep your children from this sad health progression.

At the TMJ and Sleep Therapy Centre of Memphis/Myobrace Center, we treat patients of all ages. Dominic is a young guy who was suffering from limited mouth opening, difficulty in chewing, jaw pain and he felt that his breathing might be giving him difficulty with sleeping. He came to us very tired for someone so young.

We are finding these problems and symptoms with patients in their teens, 20's and 30's. They may be having their "symptoms" treated, but the actual underlying problem has not been identified and addressed so the symptoms will only get worse.
If this is not addressed early, these young people become chronic headache and pain patients, develop Airway Breathing Disorders which will likely bring Sleep Breathing Disorders to include sleep apnea. Sleep Breathing Disorders bring a multitude of health problems that are serious if not treated properly which include Type II Diabetes and High Blood Pressure.

Another patient tells his story. Earl holds a world's record in pole vaulting (for which I am very impressed!). He was referred to us by his massage therapist (Myotherapist). She recognized the symptoms to be related to TMJ Disorder.
We educate the patient about the "why" of the disorder. We treat the causes not just the symptoms so that we get long-term resolution.
