🩺 “Are BBLs dangerous?”

Let’s addresses one of the biggest questions surrounding Brazilian Butt Lift surgery — and the misconceptions behind it.

The reality is this:

BBLs historically developed a reputation for high complication rates largely because:
• Extremely high procedure volume was happening
• Unsafe techniques were being used
• Some procedures were being performed in unsafe facilities
• Non-board-certified providers — and in some cases people who weren’t even surgeons — were performing them

That combination created serious problems within the industry.

But modern BBL safety has evolved significantly.

Today, one of the most important advances is the use of real-time ultrasound guidance during fat injection.

Why does this matter?

Because the major danger in BBL surgery comes from injecting fat too deep into the muscle.

With ultrasound guidance, surgeons can visualize where the cannula tip is in real time and help ensure fat stays in the safer subfascial/superficial plane instead of entering the muscle.

Important things to consider:

✔️ Proper training matters
✔️ Proper technique matters
✔️ Proper safety standards matter
✔️ Proper patient selection matters

When performed correctly by qualified surgeons using modern safety protocols, BBLs can be performed much more safely than many people realize.

🩺 Over the muscle vs. under the muscle — what’s the difference?

Let’s break it down.

⬇️ Under the muscle:
Can provide additional tissue coverage for patients with thinner breast tissue, which may help camouflage the implant in certain cases.

However, possible tradeoffs can include:
• Animation deformity
• Implant movement with chest flexing
• Implant displacement over time

⬆️ Over the muscle:
This approach offers several advantages for many patients, including:

✔️ No animation deformity
✔️ Enhanced cleavage potential
✔️ The option to use mesh support/internal bra techniques
✔️ The ability to perform the Motiva Preservé procedure

The best placement ultimately depends on the patient’s anatomy, goals, tissue characteristics, and lifestyle.

There’s no one-size-fits-all answer — which is why individualized surgical planning matters.

🎸 Guitar Friday — but we’re switching it up this week.

No “guess the song” today 😅

Instead, we’re recapping a moment where we had time to visit The Music Emporium in Lexington, MA. A
And wow.

Bright.
Boxy.
Super responsive.
Just an incredibly fun guitar to play.

Played a few chords and immediately understood why people love these instruments.

Then he looked at the price tag…

If you’re ever in the Boston/Lexington area and love guitars, definitely check out this shop. Amazing selection of acoustics, some electrics, and a lot of very dangerous financial decisions waiting to happen.

For today at least… self-control won.

✨ Megan’s Journey — Part 7

🩺 Why stage cellulite treatments?

A common question:

“Why not do Avéli + fat grafting like Alloclae at the same time?”

Dr. Salcedo explains that in many cases, it’s smarter to treat the cellulite first — then reassess after healing.

Step 1:
✔️ Release the cellulite bands with Avéli
✔️ Allow the tissue to heal and settle

Then after about 3 months:
✔️ Reevaluate the skin and contour
✔️ See how much dimpling improved naturally
✔️ Decide if additional refinement is even necessary

Why does this matter?

Because many patients improve so significantly after cellulite band release alone that they may not need further treatment.

And if small dimples remain?

Options like:
• Fat grafting / Alloclae
• Microneedling
• Additional refinement

can then be used more precisely and conservatively.

The goal isn’t to do the most procedures possible.
It’s to do the right amount of treatment for the best long-term result.

Sometimes less really is more.
